As far as number 4 (in your "good" column), based on what the developers were saying when that was happening I was under the impression that the scene took place directly after a "time jump." So I'm not sure if that means the companions will be constant or not. As for the rest, I largely agree except I like both Carver's looks (enough that I've switched my first playthrough choice to a mage) and Hawke's voice. I HATE the blood smear, but it's optional supposedly. It looks like he tried to paint on a fake moustache and missed.

People who are complaining about Hawke's lack of emotion in his voice are forgetting just why PCs usually are voiceless and have minimal expression. It's so you can roleplay, hence 'roleplaying game.' If Hawke was emoting all over the place, we'd be forced into that particular characterization and we could count the different personalities we could make Hawke on one hand. Or less.
Personally, I prefer no voice, because I like to imagine tone of voice, inflection, and the whole nine yards, but if a roleplaying game is going to be voiced, that voice will be neutral, by necessity. It certainly isn't because the actor can't emote--we heard him do so very well in DA:O. It's because that's what he was instructed to do to allow for different interpretations. This isn't an interactive movie where you just choose the dialogue.

So... I need to spell it out I guess. Games in Australia are more expensive than the US so you should have been able to pre-order it for less than the $60. Inn fact I''m pretty sure I remember feedback on here about pre-ordering it for way less than $60.
