You don't need to mod. The animation is part of the vanilla game, hence also on the 360.

I was thinking more of the damage caused by ramming six feet of wood through the abdomen on a regular basis making it impossible in future. Just how good is magical healing supposed to be, anyway?

Merrill knows how much Hawke cares about her, so she never uses Wounds of the Past while Hawke is looking. However, Hawke is a two-handed warrior who is often in the thick of battle. She also spec'ced Berserker and Reaver, so she's often in a frenzied red-rage.

Practically speaking, Merrill uses wounds quite often.
