well let's see. Were I writing this as a fanfic, it would obviously be for my personal canon.
But to answer both points, Hawke, Merrill, and the rest of the companions could maybe come across Ariane's clan on their way to Halamshiral, where Ariane is wed to Finn. While the clan initially disapproves of the marriage, Finn's knowledge of ancient Elven culture and his ability to translate Elven words allows them to warm up to him.
Merrill and Tobias Hawke could maybe seek asylum amongst the clan for a brief respite, and say that they need a way to escape whoever's hounding them.
Ariane and Finn happen to chime in saying they know the perfect way, and then give Hawke and Merrill a copy of Morrigan's notes as well as telling them everything they saw about Morrigan's Eluvian.
When they enter Halamshiral, they could interact with all of the Dalish Elves there. Merrill could maybe interact with some of the Elves before seeing some of the Elves from her clan. Elves that we never saw and that ran away from the clan around Act III, and she would run off into the distance with tears in her eyes. Hawke would then see Fenarel, Ineria, Ilen, and Paivel in my fanfic moving towards and they'd talk about Merrill briefly before Hawke left to go find her.
I won't spoil what they're saying about Merrill though, as that's crucial to my DAII fanfic that I'm writing. Suffice to say Hawke said some powerfully moving things to the clan that made them think long and hard.
Hawke would eventually find Merrill sitting at the top of a tree and try to get her to come down, saying everything's alright. Merrill however still feels guilt ridden about everything, to which he tells her that she shouldn't. Nothing that happened there was her fault.
It would take a while, but she'd eventually climb down, though she'd pause for a moment. She'd see something off into the distance. She'd then climb down even faster and upon reaching the ground, she would first hug Hawke and tell him that she's lucky to have him in her life.
Hawke would then look into her emerald eyes and say he's the lucky one. Cheesy? Perhaps, but the sentiment is still nice. Then he'd see something in her eyes.
"Merrill, I know that look. You saw something up there didn't you?"
She'd hesitate, but she'd stare directly into Hawke's face and say that she saw a cave of sorts. Something she hasn't seen before, and she's always spent time in the forest when she was here.
"It's... it's as if the forest wanted me to see this. As if it wanted you to be with me when I see this."
"Say no more, Merrill. I'll get Varric and everyone. We'll check it out."
After a while, Hawke and everyone would meet up with Merrill. They'd explore the cave, fighting off what threats lay inside and would find it filled to the brim with ancient Elven artifacts, tomes, sarcophagi, etc. At the end of it all lies the Eluvian of the Dales. Unbroken, functional, and non-tainted.
Merrill would walk up to it, and it would respond to her blood. It would glow, but not necessarily a glow that says "Walk through me!". It's just responding. Merrill would then cast a spell on it to find it glow even brighter.
It would be here that Morrigan, Xanthos Aeducan, and the OGB Kratos walk through the Eluvian. Xanthos, in a sense, has already met Tobias (which you can see in one of the later chapters of my DAO fanfic). Morrigan would then talk to Hawke saying that he's sewn the seeds of change in the world, and it remains unseen how the world will respond to this change. Will they fight it only to let it destroy everything? Or will they accept it and let it liberate them?
"You're just as cryptic as that old woman."
Varric would laugh and Morrigan would respond accordingly, knowing full well who he was talking about. Varric, Isabela, and Anders would then remark upon knowing who Xanthos is.
"Varric, who is he?"
"Hawke are you kidding? He's the second son of Endrin Aeducan, former king of Orzammar. The man that was framed for a crime he didn't commit and cast off into the Deep Roads to die only to be saved by the Wardens. The one who would lead the fight against the Blight after Ostagar and return home to take his rightful place in the Memories and on the throne."
Anyway, Morrigan would say that Hawke has a choice. He can continue to run away from his pursuers, or he and his bride can elude them by stepping through the Eluvian.
And... well... he'd go through with Merrill.

I don't know, that's just a rough idea of what I'd write. With a few days devoted to this idea, I could probably make it better.
Now one might think that I'm ignoring what Varric said in the endgame. But I'm not. I'm taking creative liberties
