Rad Tools is simply running Rad Tools to display the vids. In Game, you have a lot of system resources being taken up by the game itself, and then it has to load up and play the vids on top of that. Mileage may vary depending on how good your system is.

Did you try using RAD tools to check the frame rate in the movies themselves? Opening Bink and using the File Info button, it's telling me the videos (the few random ones I checked, at least) are 30 FPS. 15 for loading screens. So 40 per second is already more than there's actual frame data for, and what I'm now wondering is why the game even bothers with the 10 extra frames. Doesn't sound to me like you're missing out on anything.
If you didn't have FRAPs open, do you think you'd have noticed a difference?
That said, I'm not getting the same results. I just loaded up the game and played the suicide mission through to the start of the oculus. The loading screens did seem to be capped at 40 FPS, but in the space exterior movies that play during that sequence, FRAPs was reporting 60 FPS. Didn't play through the mission though, maybe there are ones later on you're referring to?

Prerendered cutscenes are 60 fps for me, though they don't actually look that smooth.

No problem. It was interesting to learn that the frame rate of the videos is so low. Though, "cinematic" frame rate is supposed to be in the mid-20s as I recall, so perhaps it was a deliberate choice for more reasons than just fitting it on the DVDs.
