I can't remember what leads to it unfortunatly. But it is in one of denerim's back alleys. There is a door that is unlockable but after getting all the notes or killing all 5 revenants I think, you get to get inside the door. It is a really small room, and the dude turns into another revenant. Kill him and you'll get it. The back alley is the one south of The pearl in denerim.

Next, the final pieces you can get only in the dead trenches (you sort of have to go in the room near the end of the dead trenches - just before the other side of them (before the "Ominous door" ). Up until that point, you should have gathered all parts.
Can't make it more specific though.

How to get - get quest from sergant Kyle(?) in Denerim to 'clean up' Pearl from some loud soldiers.
I did it by negotiation (read - without fight in Pear). When heading back to sergant I was attacked by mentioined soldiers. Their boss (named NPC - dont remeber the name had this cool axe).
It glows red

If you mean the Knight Commander set, I have also looked myself my friend. As it turns out, it's one of those ubar 1 piece no-set items.
As for questions for Unbound and Topsider's Honor, those quests vary in start because they are not quests till you've finish their codex, only Lore codex notes.

In the Chantry talk to Kaitlyn and agree to look for her brother. Go to her house (on the boardwalk) and head to the first floor bedroom. Talk to Bevin and ask about the sword he mentions, you can persuade him to give you the key or tell him that you will talk to Kaitlyn about buying the sword. The sword is located on the upper level in the chest (you can't miss it). You can return to Kaitlyn and talk to her, I don't believe you have to pay her but I did anyways...

Hmm, I'm not too sure, that almost sounds like somethin in the landsmeet area? I only know for sure the locations of the Warden Commander set, so it's only a guess.

In the Legion of Death room in the Deep Trenches, you have to someone wear 4 pieces and then touch the altar. A shade boss comes out.
