Executioner's Helm is a rare drop from the yellow named knight at the end of the human noble origin. Forget what he's called exactly.
He drops junk, a normal 2h or the helm. No idea on drop rate.

Ash Warrior Axe and Saw Blade are not Unique weapons, there are more then 1 of each ingame. Executioner's Helm, found in Leliania's companion questline if i remember right, a chest in the room to the left where you also find chavalier's gloves.

Drifter's Cache I believe, through the Drifter's Cache quest you get by finding 4 'Rubble' in the Deep Roads/Caridin's Cross. Click all four and you'll get a little marker taking you straight to your gloves.
Executioner's Helmet can also be aquired from Cesar in the Denerim Market district after all Crow-Quests are completo~

Sadly it is random, however some boss's or "special" chests have them in there drop rotation, best spot to farm 2 peices is the ancient dragon and her chests on mountaintop, Brecelian ruins chests all have a chance aswell but it seems low. Takes bout 15 tries per load, and remember once you load a zone ALL the chests are preloaded with gear.
Sorry I havn't updated this post, RL stuff , then got sick, then holiday's, then more RL. I'm mappign things out on my 2nd playthrough. Wish I could give various items to my toon like nwn, but the toolset for DaO is a bit dodgee..or is it dodgey..hmm. I like to not worry about gear and just enjoy the game.

Keep in mind some sets can be bought. The boots of Effort and Diligence are both bought off dwarf vendors. There is also a shield of the legion missing from your list, which makes it a 5 part set. To get it, buy it off Ruck.
True, sadly it doesn't give it an additional set bonus.
