Ok so after compleating Witch hunt on nightmare i put dragon age away for a while i created a new character today and i didnt receive the dragon bone cleaver (not really to botherd just wnated to sell it for some money) ive got all the achviemnts for dragon age has anybody else had this bug ?

Did you unlock all achievements for Witch Hunt? Some of the items only appear if you've completed other quests.

Try reloading an old character and see if the items pop up there. Maybe just need to reload.
