After talking to Mordin Solus, he mentions that Captain Kirrahe died. I
went all Paragon in the interactive comic and it assumes I took a
Renegade choice for letting Kirrahe die on Virmire? Doesn't really make sense.

The absence of Helena Blake and Conrad Verner is understandable. It's as
if you chose to not interfere in those events, therefore no morality
issue. But the case about Kirrahe cannot be dismissed as it was part of
an event locked in your path, therefore the outcome was part of a
morality decision you should've taken and here it assumes by default you
took a Renegade decision by letting Kirrahe die on Virmire.



I also noticed that Shaila (Green Asari who was under the control of the
Thorian on Feros) is not present on Illium. When you spoke to her after
you killed the Thorian, you had the choices of letting her live or kill
her. Since she's not present on Illium, then again it assumes by
default that you took a Renegade choice by killing her.

Are all remaining important events that we couldn't play out in the interative comic have been laid out Renegade by default?
