OK, the previous notation in the NAME of the forum on the index page header has disappeared, but some part of its previous existence still shows when you hover the pointed over various places, and it shows up again. I just tried finding it on purpose after it popped up when I wasn't looking for it, but no luck . .

I've looked around this forum and the list of forums, and I cannot see a notice concerning spoilers. Could you show us where this notice is posted?

1) A mod is gumming up the works. (EDIT: When I see that a game's been modded, that becomes my A #1 suspect).
2) There's still something to be done in the game-- does the codex indicate that all the quests are complete? Are there representitives from all the factions in the party camp?
3) Your saved game was somehow corrupted and you need to try again from an earlier point.
4) You're experiencing some unknown system issue that will require a reinstallation of the game.
It's hard to troubleshoot anything more without knowing what version of the game you're using, how it's patched, and what your system specs are. But hopefully this is enough to help you do some detective work. If not, then there's always EA's official help, available through help.dragonage.com.

Oh, sorry. Well, uh...

The codex say Irving is dead and the Circle went bye-bye, it says Harromont is King, it says I killed the Werewolves, and those are the quest codex.
My Journal says Gather The Army AND Tell Eamon when i'm ready. .-.
