This too could be a factor as well as I finally finished my current playthrough and I had in fact done the same thing (setting the difficulty to Insanity) as well as only playing this 1 save all the way through and first try as well - Insanity lit up.

I'll update the front post with all the things and suggestions made so anyone else trying for this trophy won't get frustrated.

Can confirm Insanity trophy is BROKEN. Set the difficulty before beginning a new game, NEVER changed it, got the best ending and every other trophy popped, NO Insanity trophy. Did it the exact same way on Xbox and it worked perfectly there. Screw this. of all the trophies to screw up it always has to be the one that's hardest to get.

Things I did exactly:

-Prior to starting the game once I first popped it into the tray, changed it to Insanity in the menu
-Never so much as checked the difficulty until I realized the trophy didn't pop. It was still "Insanity"
-It was my first playthrough
-Had NO other characters on the go

This is the worst bug to have. needs fixing. I'm a little miffed. I have a backed up save a couple missions back from the suicide mission. I'll try loading it in, beating it again, and see if it pops.

EDIT: Can anyone confirm if they know about this one? They're fixing the save game corruption and I think they know about the NG+ equipment glitch for next patch, but this one it really frustrating. It seems as if there's no one cause for it, but I'm sure if they look into it, it likely stems from the same bug.

Just beat Insanity for the first time. 16 hours. Didn't recruit everyone but kept who I had loyal. Saved the crew. Everyone survived. Didn't even need all the upgrades.

I was a Soldier btw. Oh, and all you need to do is put Insanity when you start that specific character and it gives you the option. Don't change it if you go in the menu. I played in 2 hour bursts also in case you were wondering (to prevent save corruption) and I didn't do DLC missions. This was really easy!! PSN tag= luce399

I dont think it's broken for everyone. This was my second character, first time playing insanity and I got the trophy. I even checked the menu a few times (but didn't mess with the insanity setting). This glitch doesn't happen to everyone I guess...very unfortunate that it happened to you :(

Short version (since it's beginning to get off topic here, though I'll keep is semi-relevant with the first point).

1) There's a bug with the NG+ in that all the weapons and armor you acquired within the game won't be there when you restart.  (Bioware is aware of this.)
2) The enemies do level up with you and after a certain point, they level up stronger than you do (remember, neither you nor your squad gets much in the way of ability points after level 20).
3) Coupled with 2, as you're leveling up, you are able to pick up enhancements for your weapons and abilities through research.  In a NG+, you may have Heavy Warp as an ability, but you have to re-research the enhancements (such as faster cooldowns, enhanced damage for the squad, etc.)

So those are some of the reasons why a new game can be slightly easier.

Oh, and there are DLC items to add new Weapons and Armor into the game (which since it is DLC, the armor will stay with you in a NG+ while you can access the weapons as soon as you reach the Normandy as far as I remember.)

Hopefully BioWare will fix issue #1 but I am afraid with them it's a question of 'if' more then 'when'. As much I love this company they are not very well known for their post launch support in the way of patches. They still have issues on the 360 version that they haven't addressed yet.

Thanks for the warning about the insanity run. I will hold it off until they get around fixing this.

Out of curiosity...what's the better approach to the insanity trophy....

1) Playing a brand new character at Insanity difficulty

or

2) Importing a character from a different difficulty playthrough (Hardcore) into a new Insanity playthrough?

I don't play much in the way of shooters, and after my first complete playthrough (with Infiltrator), there are a couple of missions that I anticipate becoming very difficult on Insanity and even Hardcore so I'm thinking I need to warm up at Hardcore difficulty first before doing a run through Insanity.

Granted, I also have two character types I want to try out... Vanguard and Sentinel. So I may do a brand new start when I go for the Insanity trophy anyways.

Use a brand new character, if you import the enemies will be at your level of ability but you will have no upgrades. So in other words the start of the game would be hard as hell.

Use the class your most comfortable with, I personally find it easiest with the soldier though, mattock + adrenaline rush makes it a breeze.

So I've just beaten Mass Effect 2 last night on Insanity with my ng+ save and to much of my disappointment, I didn't get the trophy. I believe I set the difficulty when I was making my Character along with never touching the difficulty settings at all. I also never switched characters. And I've done everything from the dlc missions to side missions. I don't get why I didn't get the trophy.

I've also tried restarting the suicide mission again and the trophy didn't register. Will deleting my other character with Normal Difficulty help?

Will the latest patch help fix this issue? I was really disappointed, and I'm not sure if I really want to play me2 a third time trough insanity. Man, One trophy away from getting platinum. :/

I was wondering why I died alot on insanity with my lvl 30 Vanguard lol but thats good that it got harder otherwise it be too easy, the more you play the harder it gets. I just wish the game was more like mass effect, being a spectre was hella fun especially when I got to explore more and had more dialogue in conversations. Mass Effect 2 is more of an action game then an rpg game but maybe thats just me. Still a great game

I agree, though I love Mass Effect 2 the depth of the original is sorely missed. :(

Good news is we have been promised richer RPG elements in the third Mass Effect.

Just finished the Insanity playthrough yesterday... everything went great, but cause i forgot the chart and wanted a tough team to protect me at the end, lost Mordin... still, One casualty, coulda been worse :)

Not much i can add... i barely bothered with the DLC/optional quests, just recruiting and loyalty. Now the insanity complete save is gonna be sat untouched for a good while... PS3. not gonna get to keep the Geth Pulse rifle etc. so why not just carry on and drop the difficulty.

poor Paragade femshep tho, didn't even take time for the romance stuff. not even in the backstory.

edit: random point, but on that note... those who don't like Kaidan's passport photo in the pic as femsheps, just don't pick a ME1 LI. no photo to be incorrect.

Hey what happens when I accidently press switch career (and the other save is a lower difficulty) from the start menu, but I don't actually play that game? Does this glitch my Insanity trophy? Or does it only glitch by loading and playing that lower game file? I am going to be pissed if I messed this trophy up (last one needed for a platinum trophy)Posted Image! Also, my game sometimes doesn't automatically load my insanity save. I have to manually load my insantiy profile from the menu... Is this normal? I have been spending a lot of time on insanity!

I think you are fine. But what I don't get is why do you chose to load? Cant you just hit resume or you disabled auto-save?

Trophy unlocked  w/o any problems for me, thank god. I set the difficutly in the main menu before starting NG+, doublechecking after starting the game. Had a hell of a ride with my NG+ Vanguard (Kasumi was a BIG help on the first quests). I only played the recruitment and loyalty quests and the Shadow Broker and Kasumi DLC (for the money, upgrades and weapons) before going on the suicide mission..., game froze several times on me though...

I just had this happen to me as well. When I first started the game I switched to insanity as my difficulty before I even started any characters. I made one character and when I first get up from the table I double checked and it was still on insanity. I have also never created nor played another character on my PS3, this was the only Shepard. I finished the suicide mission and everything else popped up. Insanity trophy did not. I am disappointed to say the least.

To be clear I do not have any other careers and insanity was selected from extras -> options -> gameplay -> screen before I even started. So, yes there is a problem and people aren't mistaken. It is happening to some of us.

Well, I've just finished the game on Insanity, right from the start.  And guess what...  no Insanity trophy popped up.  I'm really pissed, as it's litereally the only trophy standing between me and my platinum.  I've invested more than a hundred hours into this game and even though I loved it from start to finish I am NOT going to replay it.

Where the hell is the Bioware folks?  Why haven't they addressed this?  Are they trying to ****** PS3 owners off or what??

*Sigh*

OMG OMG :D Conrad Vernar replies: Really? lol but yeah more RPG element is always welcome, hopefully the lvl cap is higher this time lvl 30 felt a little short.

Question for anyone not getting the trophy. Were you trying to attempt Insanity without having imported a character or on your first run? I may be grasping for straws but maybe there's a problem with it being unlocked when grouped with the end game trophies.

Well, what I did exactly was play normal first with soldier finished normal, I didn't import my character because the game gets harder if you play insanity with a lvl 30 no upgrades and all the enemies are alot harder to kill. So I just played a different class picked insanity and completed it, end of the game I got my insanity trophy. I took my time doing this though. I started completely new lvl 1 new class and ended up being lvl 30 around the suicide mission. I'm sure theres other ways but yeah thats how I got mine.

All I know is that I played the game for the first time on Insanity, with the difficulty set at the main menu prior to starting. I played the entire game without changing the difficulty level at all. I invested more than 100 plus hours into the game. By the end of the first playthrough I earned every single trophy except the level 30 with one character (I was level 29; 300 xp short of 30) and the Insanity trophy. It is obviously not an isolated issue. Bioware either missed something or there is a legitimate BUG in the game with respect to this trophy unlocking. I am afraid to play through the game again. Not only for the sheer number of hours it'll suck from my life, but also I seem less than confident that the glitch won't repeat again.

BIOWARE! GET YOUR HEAD'S OUT OF YOUR ASSES AND PATCH THIS GAME ALREADY!
