After I escaped the Ghost ship, I heard the ding and the bronze Ghost Ship trophy notification appeared on the screen but when I checked the trophy progress terminal in Shepard's Quarters, I noticed that the Ghost Ship trophy is blacked out.

I had similar situation, however it did show up later on. I think I know when but I forgot to check it at the point I assumed it might do. Have a look after you visit another derelict ship (main mission one, not a sidequest one)
