We may need a little more info to help troubleshoot. What version of the game do you have? And is it a physical disc or Steam/other digital download?
Also, do you have any mods installed?

After a bit of playing alot of the character heads are gone. Not sure where I can see what version it is, but it is digital copy from steam. Dlc witch hunt gotten with bioware points. And no mods on the game.

Hmmm... Is it the Ultimate Edition or the original version of the game released in November '09. And could you please post your system specs... Windows Version, CPU, RAM, Graphcis card and video Ram, stuff like that?

I have the original and awakening showing on my steam. Both are working.

Okay... that helps. Here are a couple of options:
1) Let's make sure your graphics driver is up-to-date... I think Nvidia has released 266.58 recently.
2) I suggest trying Witch Hunt at a lower resolution... at least to test and see if that helps.
3) Your GTX 260 card is may be one of the "factory overclocked" models... which means it comes like that out of the box. Unfortunately, DA doesn't seem to like overclocked GPUs... especially when paired with an older CPU like yours. More info in this thread: http://social.biowar...8/index/5574840 This problem can cause render problems...
4) This one is a little more esoteric, but your page file size is very large, and that may literally be squeezing the usefulness out of your system memory... see http://social.biowar...8/index/5816873
5) Finally, I suppose the game may have become corrupted somehow... which would require a reinstallation... I suggest we hold off on that theory until all else fails.
