On my last suicide mission:
Miranda - died after Reaper battle (not loyal)
Jacob - died after Reaper battle (not loyal)
Mordin - died during escort (not loyal)
Kasumi - poor leadership at the 2nd set of doors
Jack - carried off by seeker swarms.
Tali - got a missile in the face (not loyal)

Vents: Legion

1st Fire Team Leader: Miranda

Biotic Shield: Samara

2nd Fire Team Leader/Distraction Team: Garrus

Escort: Tali

Final Team: Miranda & Thane

I got through without losing anyone on my first playthrough, but didn't have Zaeed or Kasumi. Kasumi's DLC wasn't out yet and for whatever reason I didn't bother to download Zaeed until my second run through the game.

Or have Shep make dumb decisions and get people killed.

Neither can I. The problem I had with ME2 is that in order to lose squadmates you have to make bad tactical decisions or skip content so that your squadmates aren't loyal. Hopefully in ME3 you'll lose a couple squadmates even when you make the right tactical decisions, or maybe even because of it. (putting the mission before the man, so to speak)

It's possible to lose some team members without making terribly bad decisions - e.g. vents: You can choose Tali (recruited at gunpoint), Mordin (recruited at gunpoint), Garrus (recruited at gunpoint) or Jacob (volunteered both for the mission and the specific task). Or how about the biotic specialist? Miranda tells Shepard that any biotic could do it, and Jack is just a little bit unstable.

No casualties on the 1st playthrough, though I lost half the crew. I activated Legion after the ambush. I agonized over some of the decisions, though I picked Legion without a second thought.

In all three of my playthroughs, no one has ever died. Honestly I think they overhyped it too much, as long as there are internet walkthroughs... my team will live.

On my first run through, I lost Jack's loyalty during the confrontation between her and Miranda. Feeling that Jack was a colossal loose cannon with a gigantic chip on her shoulder, I sided with Miranda. During my run through, she died holding the line because I took Garrus and Grunt into the final fight with me.

On my subsequent two run-throughs, I lost no one even when Jack still wasn't loyal due to the same confrontation. I did leave two heavies with the line team though.

On my fourth playthrough, I just didn't feel like dealing with Thane, so he wasn't loyal. I took he and Miranda with me to the final fight and the falling debris killed him.

I always send Legion through the vents.

I always use Samara for the biotic specialist.

Garrus is always my fire team leader; now always leader for both fire teams.

Jacob always takes the crew back although I'd send Miranda if I could.

Is it possible to do a playthrough where only Zaeed and Morinth survive?

I've been running the numbers in my head and the only way I can think is if you got Morinth AFTER the suicide mission.

Deaths:
Jack: No armor
Legion: No shielding
Thane:No cannons
Garrus:Tech death
Grunt:Killed by swarm because of failed biotic (Miranda would be that Biotic maybe unloyal samara)
Jacob: Second fire team leader dies
Tali: Dies escorting

Loyal Zaeed and Unloyal Miranda fight main boss Miranda dies

Samara, Mordin, Kasumi hold the line Samara's defense score is 1 and only she should live.

Then do Samara's Loyalty mission and get morinth.

If the above will work is it possible to bring Morinth with you to the suicide mission and still only have Zaeed and Morinth Survive?

Yes, you can do this, but you'll need to fiddle with the order.
Lose Miranda's and Tali's loyalties beforehand.
Jack will die from lack of armor, Legion will die from lack of shields, Thane will die from lack of armor. Choose Jacob for the vents to kill him. Bring Garrus and, say, Kasumi along with you and choose Miranda for the biotic; Garrus will die. Have Grunt close the second door and he'll die. Choose Zaeed to escort the crew; he lives if he's loyal, and the important thing is to get rid of him here. In the final battle, bring Miranda and Tali, and both will die; Mordin, Kasumi and Morinth will hold the line. Mordin and Kasumi will die, but Morinth will survive, and Zaeed will be waiting for you at the ship, so fly away.

On my first play through I lost Miranda and Zaeed. I picked them for the final showdown because they were the only ones not loyal. I figured my loyal crew could hold the line and I would protect the two I did not get loyal. How wrong was I? They fought bravely and died as goddamn heroes. RIP.

Call me curious but I'm intrigued as to what you mean by 'recruit at gunpoint'? I mean Tali in ME was a definite 'railroad' recruitment but am intrigued as to what referring to.

Speaking of Tali and her 'railroad' recruitment this is another point where 'bad tactical decisions' aren't necessary for people to die. As I said, in ME you were forced to have Tali along, in ME2 you weren't, thus ensuring the death of one of your Squadmates without Shepard knowing that Tali would have the wonderful Ship Shield upgrade that would save a life. Some people might not have bothered recruiting Garrus in ME and thus didn't really trust him or want to talk to him much even though they were 'railroaded' into recruiting him in ME2. So you could easily have him or someone else dead by not talking to him and finding out he has info on a cannon upgrade. Then of course there is Jacob. Aahh Jacob... yes because even though you don't trust Cerberus your willing to talk to him and find out that he has contacts who can get an Armor upgrade that will save the life of Jack.

So this talk of 'My Shep doesn't make bad decisions and I don't meta-game and so I don't lose people' sure, might be for you, but it doesn't mean to say it is the only way you can get people killed off. Also if you don't pay much attention to Kelly Shep could easily not know that some of the squad aren't focussed even if of course it is brought up after the collector ship mission AND also after the abduction in the scene with Jacob, Miranda and some others and so you could as Alex mentions above, send Tali to her death because you didn't help her with her trial but figured she was suitable for the job.

More back on topic and to some extent highlighting something I previously mentioned. On my original playthrough of the game on PC, I lost Thane and Jacob, Thane because of no shield upgrade due to not recruiting Tali and Jacob because I didn't have a 'Tech Expert' and as he had volunteered, my FemShep wanted to give him a chance to prove himself considering she wasn't too sure about him or Miranda. I later redid this playthrough when Kasumi was available and as Kasumi had already proven herself as a Tech Expert to Shep during her loyalty mission, she was chosen instead. Thane still died because by that point it was 'Team Infiltration' for the most part of the game with my Infiltrator FemShep, Garrus and Kasumi.

On my first playthrough on PS3, I lost Miranda, Shep had sided with Jack during the catfight as disliked Cerberus and didn't trust Miranda to remain with the others holding the door, so brought her along, gave TIM the middle finger, Miranda resigned but alas of course because she wasn't loyal she didn't spot that lump of rock crashing down on her. I think it really made the coffin scene more poignant the fact that Shep had been proven wrong about her. In this playthrough, Jacob earned Sheps trust and ended up being used as the Team Leader on both occasions.

No-one died on my first playthrough. But I I think later discovered the only reason half the crew survived was because of pure luck. I kept looking for one last mission to do before the suicide mission but couldn't find one. After I got back I saw the Geth weather manipulation mission was left, so I guess Kelly is just lucky Shepard can't find things that are right there on the galaxy map.

It is a bad tactical decision to assign tasks to anyone who isn't loyal, because the game continually reminds you that you need everyone loyal, or you are going to lose people. Jacob in particular gives you a reminder in conversation about the Normandy, if you ask him how he feels about the squad.

Jack can successfully defend the squad from the seeker swarms if she's loyal.

While Miranda suggests that any biotic could in theory, hold off the swarms, Jack and Samara are described as being extremely powerful biotics. Miranda has biotic abilities but unlike Jack or Samara she's never portrayed as being a ridiculously powerful one. So long as Jack or Samara is loyal they are obvious choices before Miranda, as they're both portrayed as being top tier biotics.
