On my first, day-of-release, blind playthrough I had nobody but Jack die but I didn't do her loyalty mission so that kind of made sense.
I strongly dislike Jack and was actually quite glad that she died - I continue to let her die on my later playthroughs and will be importing that into ME3.

That may or may not be true; Shepard can't see the loyalty spotlights on the squad selection screen, and the loyalty flag doesn't, exactly, measure loyalty.
In any case, bad tactical decisions are not necessarily unrealistic ones (e.g. heroic sacrifice at the start to save Joker who'd probably have been executed for disobeying orders anyway)

And then not only do you lose your best friend to seekers, you lose your battle buddy who you gained loyalty early on, like Pre Horizon. Ever since you put that gun to his face and gained his loyalty you two were inseparable in fights. You killed lotsa mercs together and figured he can lead a team operation after all the time working together. And finally your XO dies because earlier on you didn't feel like taking BS from her. She was petty and she was being a bully. So you didn't bother talking to her ever again after you sided with the psycho who you may have to kill later before she goes on a biotic rampage after this mission.

First time I tried to keep everybody alive, but ended up losing Jack (didn't leave a strong enough force Holding the Line).
In my canon import:
- Kasumi died trying to seal the door
- Samara was snatched by the seeker swarms
- Thane was mortally wounded during the distraction attack
- Jack was killed by the falling debris
It felt much more like a suicide mission with good people dying at every step.

Samara was killed off in her loyalty mission because I felt like she was an annoying, self-righteous b****, then I picked Morinth as second fire-team leader so that she'd die because I'm not letting that murderous wench go free.
Going to do a few more playthroughs where I'll let several people die during the suicide mission, mostly for sake of drama (got plenty of 'everybody lives' playthroughs for import to ME3 anyway)

Obviously I have multiple playthroughs. Many with the entire crew alive and loyal.
Female Shepard killed,Kaiden,Thane,Jacob,and Garrus.Paragon.
Male Shepard killed all male crew,while saving an all loyal female crew. Paragon.
Male Shepard got everyone killed except Tali & Grunt. Renegade.

Apart from RP, I've never seen any sense in selling Legion. You sacrifice XP (from his loyalty mission) + a squadmate for 50.000 miserable credits. It should have been at least 100.000 for it to become a little tempting (even more having in mind that you can recover up to 30.000 credits in his loyalty mission).

I only played the game twice and that was because I keep on getting stressed to get everyone through XD
But on my first play through I got everyone through. All except Mordin. I'm not sure why he even died in the first place. I had his loyality and I never chose him to be a leader in any of the choices.
On my Second playthrough I ended getting everyone, and managed (luckily) to do it without Jack's loyality. Was paranoid since I couldn't resolve the issue between Miranda and Jack earlier on that playthrough.

Lost no one on my first try because I'm just that awesome. To keep everyone alive think about their backgrounds, experiences and personalities:
Fire Team Leader 1: Garrus (as he led his own squad on Omega) or Miranda (been a leader in Cerberus operations for a while, knows how things work.)- Don't know why Zaeed wasn't a suitable option though.
Tech expert: Tali (She's a Quarian, they're masters at techs) or Legion (It's Geth, he IS technology) or Kasumi (master infiltrator and hacker, which questions why Thane isn't good.)
Biotic Shield: Samara (1000 year old Matriarch Justicar, obviously very powerful biotic) or Jack (The walking experiment on mega biotics) Strangely why isn't Jacob good at this? I know weird but his loyalty ability is "Barrier" so surely he could stretch that a bit further in SM?
Fire Leader 2- Same as #1

I didn't recruit Tali or Thane in my last playthrough, so the Normandy had weak kinetic barriers and Legion died before reaching the collector base. I had no tech expert but Jacob volunteered for the job in the vents, so I let him do it. He did a good job before he died. Miranda and Garrus were my leaders, Jack was responsible for the barrier and Grunt took care of the crew (he looks like a good bodyguard anyway).
My squad after the mission was Zaeed, Samara, Mordin, Garrus, Miranda, Grunt, and Jack.

First time around I lost Miranda because I sided with Jack in their argument without the points to defuse it. Had I just used a different squadmate to do the suicide mission with she would have survived, but I hadn't done any research beforehand and didn't know any better at the time.
The second time I had everything figured out so no squadmates were lost, although I did lose my crew minus the Doctor because I did too many missions after they were abducted.

On my first Suicide Mission, I lost the crew. As the other tasks all needed a specialist, I was trying to think of a squad member who was uniquely qualified to defend a large group - I couldn't decide on anyone. I was so paranoid about losing the squad that I didn't send an escort (I know; I suck). Only after the mission did I find out that any loyal member can successfully defend the crew.
Since then, there've been zero casualites in my playthroughs. For my Spacer/Sole Survivor import, I'm planning on losing a few people on purpose; solely for role-playing reasons.
