none...everyone was loyal, i did all the upgrades, and i was smart/lucky with my choices...used samara for the bubble, tali to unlock, mordin to protect the crew, garrus lead the other team, and i took legion and kasumi with me to fight.....i say i was partly lucky cause i later found out about the whole defense/points thing that could affect the outcome...if i knew about it before hand, still wouldnt have changed anything....garrus was my right hand man, mordin was my trusty killer comrade that handled business, kasumi was my non cheating 'cheat code' killer (also my <3, i loved that chick/character), and legion was a straight killer with the widow...

i jinxed myself...playing through my ultimate play of ME (meaning starting from 1), i just finished this mission and the crew died lol....i forgot that once you get access to the go through the omega 4 relay, you're suppose to go through as soon as possible, or the crew will be affected...everyone else lived though

First time I did it I screwed up big time, Mordin died at the door, Miranda died fighting the human Reaper as she wasn't loyal to me and Kelly got juiced as I took to long to come rescue them, whoops!

2nd Paragon playthrough went more smoothly, everyone was loyal so everyone lived. In my Renegade playthroughs though I always kill off Jacob.

No one. I was disappointed how easy it was.

For me Thane died. Everyone else lived.

I had Thane go through the vents because I knew he had previous experience going through air vents when I met him and he knew how to sneak around and not get caught.

Garrus as the second team leader both times

I used jack for the biotic barrier because she is a very strong biotic.

I used jacob to escort the people back. It just seemed the most fitting job for him.

1st Fire- Garrus
Biotic Bubble- Samara
2nd Fire- Miranda
Escort- Mordin
Final Battle- Miranda and Kasumi

I guess those are my usual choices, but some are subject to change.

Finished ME2 for the first time recently (yeah, someone's a bit late). Hadn't looked at any spoilers.

Only Jack died (well, besides half the crew) since I forgot to get that one upgrade for the ship. Fail. She was also disloyal because I didn't have enough renegade/paragon points to solve the fight between her and Miranda without taking sides. I valued Miranda's skills more so... yeah. Tali was also disloyal thanks to her little fight with Legion but she survived. 

I sent Legion to the vents.
Assiged Jacob as the second team leader. I thought Miranda could cause aggravation (+ I wanted her with me).
Samara was my obvious choice for keeping up the biotic field.
Garrus as the leader of the second team.
Jacob as the escort

My personal team was Thane and Miranda through the whole mission. Pretty much through the whole game .
