(Go immediately after crew abduction)
No armor: Jack dies.
No shield, take Kasumi and Legion to fight Oculus: Tali dies.
No cannon: Thane dies.
Specialist: Jacob (dies).
Fire Team Leader: Irrelevant.
Escort: Samara (must be unloyal, dies)
Biotic barrier: Anyone (squad = Mordin, and Kasumi or Zaeed. Mordin dies (note, Jacob and Mordin are interchangeable in this scenario)
Then you're just left with Miranda. Either leave her behind in "Hold the Line" without two of Zaeed, Garrus or Grunt, or take her with you to the Huma-reaper. Either way, she must be disloyal.

I want as many new squadmates as possible for ME3 on one of my playthroughs.
And what about only having the ME1 squaddies survive, Garrus and Tali. How can I keep just those 2 alive?

Keeping just two alive can be tricky. So just a friendly FYI to anyone reading the thread: It's a little late to plan something like that when you're just about to launch the suicide mission.

Never said it (Samara + Zaeed surviving the suicide mission) couldn't be done.maitreikrit2 wrote...

so...what happens if zaeed is your only squad member and then u do his loyalty post-suicide and leave him for dead?...
1) You'll never be left with Shepard alive with just 1 squad member after the SM.
2) If you only have two (Zaeed included) left, you won't be given that option with Zaeed.

So worth it.
Thanks, Hah Yes Reapers, for reminding me that a unloyal escort dies.

When you first land on the collector base, after you have selected squad leader and vent specialist and such. What determines who walks out of the Normandy behind your squad leader and specialist? Is it random?
In my experience, Thane is usually in there... I think it's a pecking order, perhaps the same one as who dies without the Thanix Cannon, because I once put Thane as vents and Garrus was following.

It's possible if (for example) you don't have Kasumi and Miranda is not loyal.

Yes. No Kasumi, Miranda is not loyal, no crew left. Only survivors being Tali, Legion, and Zaeed. Possible?

yea i think you're right, there is an order. Thane being first and Jack always being second, not sure after that tho.

It's probable that you will just not be able to import a save where you died. Boring as that... But hopefully, BioWare will surprise us.
they have said it in one of the interviews that if ur shepard died... its game over. Galaxy is lost and stuff.

Yar, just like the failed mission clip in Arrival.

5. Will the squadmates hold the line?
Pick two out of the three following Defenders to leave behind = [Survived!]
Any other combination = [Death] (Or is it?) (Priority List - unloyal squadmates die before loyal ones)
Does this mean that picking all three may result in someone dying?
If so, it should be clarified in the post quoted above, because it stands to reason that if two of Garrus, Zaeed and Grunt can save everyone holding the line, then three should as well.
If not, then there's something wrong. I had Grunt, Zaeed and Garrus holding the line, all of them loyal, along with several squadmates who weren't loyal. Mordin, not loyal, died holding the line. I had loyal Samara and loyal Kasumi in the boss battle, thought that shouldn't be of relevance here. Correct?
(My intention was to let everyone survive with a minimal number of loyalty missions, but on the other hand having someone die makes the mission a bit more meaningful and it brings variation into ME3 once the save is imported.)
