One way to ensure that no casualties will happen is to get rid of the weaker characters (Mordin, Kasumi, Tali and Jack). Have one of them taking the crew and the colonists back to the ship, and take 2 others with you, leaving only one of the weaker behind.

Having a loyal Zaed and a loyal Grunt can improve things quite a bit during the hold the line section. Your chances also increase if you have Grunt and Legion recruited and activated, and the 2 DLC characters Zaed and Kasumi.

Once playing the pc version, there was a bug with me that let Miranda die while whe were running from the seeker swarms. Strangely, she was alive and well in the next cut-scene. A similar bug appears if I bring her to fight the intruder. She speaks with me thorough the intercom, but she´s right at my side. To avoid this annoying situation, I never bring her with me.

When I want to ensure that everyone dies, I try to eliminate first the members with more potential to survive the hold the line segment.

Therefore, I´ll always that Kasumi and Tali to the intruder section, so that both Thane and Garrus will be killed. After that, I kill Jacob in the infiltration. Then I try to kill Grunt and Zaed at the same time, taking the strongest character remaining with me to ensure their deaths (legion, miranda and Samara). I never take mordin, tali or kasumi with me nor they to kill any of them earlier, because they degrade the hold the line score. If you send someone to escort the doctor, it´s even easier. If sending legion to cerberus, waaay easier.

The one thing I dislike about the Suicide Mission is that it is way to easy to get everyone loyal if you just play the game. If you rush through it, you have troubles. It rewards the completionist.

Well, that´s the way to do it, isn´t it? It´s effort being rewarded. If you rush in, you´re careless.

It just makes it way to easy for the player who knows what they are doing from the start.

This is what I just did with my latest Shepard. I call it "Kill all male squadmate" trilogy playthrough.

Honey Shepard has just completed the suicide mission.

Garrus died because the Normandy had no shields

Jacob died in the vent because he volunteered for it and I wasn't going to deny him from doing it

Jack escorted the crew back to the ship

Had Thane do the barrier which allowed Grunt to be taken by the seeker swarms

Zaeed was killed as 2nd fireteam leader

Took Samara and Miranda to fight the reaper and lived.

I had Thane and Mordin, both unloyal, holding the line and surprisingly, Thane survives. He will pay for that in ME3.

Jack, MIranda and Samara are going to ME3 and Kasumi and Tali will join them when I recruit them.

Last thing. I gave the Geth to Cerberus.

So now, Honey Shepard has to worry about killing 3 male squadmates instead of 2, no thanks to Thane, when she starts ME3 later today.

Now my question(s) are How is it Thane was able to survive? I did the same except I had Jacob and Mordin holding the line and Jacob survives.

Now I had Jacob escort the crew back instead of Jack leaving her to hold the line with Mordin and both died even though Jack is loyal. How is it she dies and Thane or Jacob survive, if both are unloyal, holding the line? I also did this with Samara, who was loyal, and she to died. So, I'm just what-the-heck is going on. But that's alright because I know at the end of ME3 all male squadmates will be dead which is the goal for this trilogy playthrough.

Thane = 1 point, Mordin = 0 points.  2 members for the HTL team. Average = 0.5 points. 0 < Average < 2 = death of one HTL team member.

I've come to the conclusion that the formula doesn't cover every possible scenario.

Can it explain why I had Mordin, Tali and Miranda, all loyal, but still died when holding the line?

Mordin = 1, Tali = 1, Miranda = 2. 3 members HTL team. Average = 1.33333. 1.0 <= Average < 2.0 = death of one HTL team member.

It still doesn't explain why Jacob and Thane were able to survive being unloyal whereas I had Samara loyal and she dies. Or explain why I had Miranda, Tali and Mordin, all loyal, who all died.

This is weird. It must be a bug, since as a rule, all non-loyal must die first. Sometimes the suicide mission bugs during the biotic corridor segment on my pc version and the wrong crewman dies because of a failed check on the flags of which members were with me.

I thought I remembered seeing a different version of the Hold the Line math several years ago, but it has been so long I don't really remember.

I didn't try to test it or scheme to get just the right squad members to die though.

I would like to add my thanks to everyone that helped put this together. I've always played the game to get everyone loyal and survive the SM.

Thanks to this I've decided to play a not so good play through. I would like to see how ME2 and ME3 play out with heavy casualties. So Wrex died in ME1 and this is my plan for the SM, I hope I've figured it out correctly, feel free to point out any mistakes if I've made any.

I plan on fully upgrading the Normandy so nobody dies on the approach. I will send a loyal Legion through the vents, I want to destroy the heretics so the Quarians are stronger in ME3. An unloyal Miranda will lead the support team so Legion will die. Miranda will then generate the bubble for Shep, Tali and Grunt, resulting in Grunt's death. An unloyal Zaeed will lead the second support team and also die. I will send an unloyal Mordin to escort the crew and he will die. I will take a loyal Garrus and an unloyal Thane on the last part with Thane dying. The hold the line will be loyal Tali, Jack and Kasumi along with unloyal Mirand and Jacob. Each of those are worth 1 point which results in 2 deaths. Those would be Jacob and Miranda since they are not loyal and are the first 2 on the list of how dies in which order.

Have I calculated this correctly?

Thanks!

I don't want to just make obviously poor decisions just to kill people off. I decided who I wanted to survive and went from there, wasn't easy though. This way I figure Shep made good choices but things just didn't go their way. Plus as cocky as Miranda is and how she said something about how bad things go when she does make a mistake I could let her screw up a couple of time resulting the the death of someone before dying herself.

If you use Miranda for the barrier its not a guarantee that Grunt will die.

This is what I would do.

Have Thane do the barrier with Miranda and Grunt guaranteeing Grunt's death. Send Garrus back with the crew and he survives. Have Jacob be the 2nd fireteam leader and he dies. Take Miranda and Thane to fight the reaper and both will die. Zaeed, Tali, Mordin, Kasumi, Jack to hold the line. Mordin will die for not being loyal and if Zaeed does live, you can kill him during his loyalty mission

Actually looking back at my original post I see that I left off Samara. She will be unloyal and holding the line. So if I use her for the bubble and bring Miranda and Grunt then Grunt will die, correct?

Yes.

If you want you can recruit Samara after the suicide mission and when doing her loyalty mission side with her daughter, Morinth who kills Samara and Morinth will be killed in London in ME3 as she has become a Banshee.

Its possible Samara might live even being unloyal. So if you have her in ME3 you can see her on the Monastery mission and let her kill herself or if you don't do that mission, her name will appear on the memorial wall after you finish Chronos.

My lazy explanation is that everything was tested on the 360, and the PS3 was coded afterwards and likely doesn't follow the same rules.

I just finished doing a playthrough killing all female squadmates.

This is what I did

I had Morinth kill Samara during Samara's loyalty mission

No armor upgrade killing Jack

No shield upgrade killing Kasumi

Tali died going through the vent because she wasn't loyal

Morinth died from the seeker swarm because I had Thane to the barrier and Miranda with me

Miranda dead fighting the proto-type reaper

And I was thinking you were going to be happy cause traynor would be the last woman standing...

Now I guess she will be the only woman standing... alone.

Yes she will. I expect when done ME3, the memorial wall scene will be Kaidan, James, Garrus, Steve, Joker, Javik watching Samantha putting up the Commanders nameplate.
