The other thing is, the second oculus or it might be the same one, doesn't come through the hole already made in the Normandy, but makes another hole to enter.

What I would do is do their loyalty missions. Besides those 3, have Miranda, Jack, Jacob, Thane and Legion on your squad. Get armor upgrade and no sheild and thannix cannon upgrade. Take Garrus and Tali to fight the oculus and Legion will die having no shield upgrade. Thane will die with no cannon upgrde.
Send Jacob through the vents. Send Tali back with the crew. Have Garrus be 2nd fireteam leader, Miranda holds the barrier and bring Jack and Grunt with you leading to Jack being taken by the swarm. Send Miranda and Garrus to fight the reaper where Miranda will die. Grunt will hold the line by himself. This is the only way I can see it being done since Miranda has to be alive to either hold the line or fight the reaper

My top priority is to have Grunt hold the line by himself. Everything else is just details, heh. My original plan had Jack, Tali and Thane die from no upgrades. Jacob dies in the vents, Legion dies as 2nd fireteam leader, Samara gets carried off by the seeker swarm while Miranda does the bubble thing. Garrus escorts the crew, and Miranda and Mordin accompany shep to fight the baby-reaper, leaving Grunt to hold the line. Miranda, Mordin, Grunt and Garrus survive the mission, and I recruit Zaeed and Kasumi afterwards.
However, I've been considering a way for Tali to survive the mission, either in addition to the others, or as a replacement for one of the above. If I take Legion and Tali to fight the oculus, Thane dies from no shield upgrades, which means Garrus dies from no weapon upgrades... which would be fine, kinda. I was hoping to keep Garrus alive if possible.
If I upgrade the weapons to keep Garrus alive, and I don't recruit Samara -- would mean backtracking in my game just a little bit -- then Mordin could take her place during the long walk, and I could take Tali and Miranda to fight the reaper.

Since you can put off recruitment/loyalty missions until after the SM, I would consider a way of doing the SM where Grunt holds the line without having to kill off lots of other folk.
1 squad member saved by sending them back with the crew
2 squad members saved by having them loyal and with Shepard for the final fight
1 squad member (Grunt) holds the line and lives
This leaves 4 squad members that will die - vent, second fire team lead, bubble, and one on the approach run.
I would say good candidates for dying are:
Mordin - Wiks is a good replacement
Thane - his scene is kind of dumb in ME3
Jacob - he adds nothing to ME3

Both Jacob and Jack were left to hold the line and both were unloyal, but yet Jacob survives

Zaeed and Miranda were the only ones loyal
Jack and Garrus died because I hard no armor and shield upgrades
Kasumi died doing the vent
Zaeed was sent back with the crew
Mordin was 2nd fire team leader
Jacob did the barrier which led to Grunt being taken by the swarm
Jacob and Miranda fought the reaper with Jacob dying
Mordin was the onely one holding the line. He died
Zaeed and Miranda lived
The reason why I posted the above was because I got dialogue from Jacob I never heard before and someone might want to try this
The last couple of days I wanted to see if I could have everyone survive with the fewest squadmates loyal. I know I needed at least 4 to be loyal. The vent, barrier, fireteam leader and escort.
I recruited everyone. Miranda, Mordin, Kasumi, Jack and Zaeed loyal the first time and Tali ended up dead while holding the line
I did another playthrough with Miranda, Tali, Jack, Kasumi and Mordin loyal only to have Jacob die while holding the line.
I reloaded an earlier save and did Jacob's loyalty mission. Now I have 6 loyal squadmates. This time Garrus died while holding the line. ok
I reloaded another earlier save and did Zaeed's loyalty mission. Now with 7 squadmates loyal, everyone survives
