Will Shepard die if there are only two non-loyal squadmates and both are taken to face the Reaper?
No.
It will be either be a squadmate that held the line or the squadmate that escorted the crew back. I have no idea how its determined who that squadmate that helps Shepard into the Normandy
I just finished the suicide mission to see how many squadmates can die if all are loyal. Its 7. What I did is kill off the strongest to leave the weakest to hold the line
Jack, Legion and Thane dead because of no upgrades for the ship
Garrus died being sent in the vent
Samara was sent back with the crew
seeker swarm took Grunt
Zaeed died as 2nd fireteam leader
Miranda and Jacob survive fighting reaper
Mordin, Tali and Kasumi left to hold the line. Only Mordin died.
Miranda, Tali, Kasumi, Jacob and Samara survived.
I just realized the link for the flow chart in the first post isn't working anymore. Anyone know why? I downloaded a copy awhile ago and could link it, but I don't want to if the creator didn't want it or if something from the flow chart is wrong.

That's odd. I thought Kasumi always died if you did not upgrade the ship's shields.

I took her to fight the oculus leaving Legion as the next one in line to be killed without having any shields

It is funny how you could do the suicide mission with zero casualties (even the crew) with only 3 loyal squad mates. Ex: Legion, Garrus, Samara
If you recruited all squadmates, that would not be possible. If anything Mordin, Tali and Kasumi would most likely die holding the line.
I've tried a playthrough to see how many loyalty missions need to be done for all to survive. It was 7 that need to be loyal. Here's the details.
Send Legion into the vents and have Garrus be the second squad leader. Have Samara put up the barrier with Garrus leading again, and have Legion escort the crew back. Then go with Samara and Garrus to face the human reaper. Just make sure Zaeed and Grunt were recruited. Done. No deaths.

You can have two of those three holding the line and still have someone die. It happened in the link I posted. It also depends on the number holding the line and who's loyal. For all to survive, each has to have an average of two. If not, someone will die starting with the one's that are not loyal and then with the weakest.

Add that his character growth is pretty much learning to hate races and he has no interests beyond killing things I'd say there's plenty of justification for considering him an evil character.
How is Grunt evil? I just don't see it. He's no worse than Ashley. His behavior is krogan-like but if you play ME3 you know he's a devotee of Shepard and all-in to defend races other than krogan or humans.
