In my opinion doing LotSB before the suicide mission can make sense, but not that late in the game... But "it's just my opinion though", you can do what you want.

Survivors: Commander Shepard, Entire Crew, Garrus Vakarian, Tali'Zorah vas Normandy, Grunt, Jack, Mordin Solus, Thane Krios, Kasumi Goto, Legion, and Miranda Lawson.
Casualties: Jacob Taylor, Samara, and Zaeed Massani.
Alignment: Paragon Shepard
Loyalty Missions: All Loyalty missions complete before activation of Reaper IFF, but did not earn back Zaeed's loyalty through dialogue. Sided with Jack during the loyalty struggle, won back Miranda's loyalty through dialogue. Peacefully got through Legion and Tali's struggle.
Normandy Upgrades: All Normandy upgrades purchased (hooray for not looking like an idiot on The Approach).
The Vents: Send Loyal Jacob through the Vents, this will result in his death by missile at the door.
First Fireteam Leader: Loyal Miranda. (Doesn't matter, but I chose her to show her that I do still respect her as a member of my team... I know, that's a bit extreme, but it makes sense in the story doesn't it? Also adds for dramatic effect when it is Jacob who dies under her protection.)
Biotic Barrier: Loyal Jack. Nobody dies.
Second Fireteam Leader: Loyal Samara. Samara dies leading the second fireteam.
Crew: All of the Crew survives, due to not doing any missions upon activation of the Reaper IFF.
Escort: Loyal Grunt escorts the crew back to the Normandy, he and the crew survive.
Final Battle: Take Loyal Garrus and Loyal Tali, both survive (having both ME1 squadmates really makes the Final Battle a throwback, especially since I had those two with me for the final battle in Mass Effect 1).
Hold The Line:
Disloyal Zaeed 3
Loyal Legion 2
Loyal Thane 2
Loyal Miranda 2
Loyal Kasumi 1
Loyal Jack 1
Loyal Mordin 1
Hold The Line Score: 12 divided by 7 which = 1.7... So Zaeed is the only casualty of Holding The Line.
And so there you have it! Again, final result will be:
Survivors: Commander Shepard, Entire Crew, Garrus Vakarian, Tali'Zorah vas Normandy, Grunt, Jack, Mordin Solus, Thane Krios, Kasumi Goto, Legion, and Miranda Lawson.
Casualties: Jacob Taylor, Zaeed Massani, and Samara.

Fully Unupgraded Ship Killed: Jack, Tail (I took Kasumi to fight eye thingie, but in the end, shouldn't matter who dies there), and Thane
Vents: Jacob (dies)
1st Fire Team Leader: Grunt
Biotic: Miranda (team w/me is Zaeed and Grunt...Grunt dies)
2nd Fire Team: Samara (dies)
Survivor Escort: Garrus (dies)
Hold the Line: Zaeed and Mordin (don't laugh...Mo won't die)
Brought to Final Boss: Kasumi and Miranda (both die because not loyal...my Shep got pulled into the ship by Zaeed)
So that's how I finally made sure Miranda died. Not loyal and taken on final boss fight = death (a fact that took me three ending playthroughs to get, but once I did, I got the results I wanted
Good luck taking out Miranda :-p

It is not difficult, but impossible to the extent they game will 'cheat' her into survival. She must be disloyal and can only die during Hold the Line and against the Baby-Reapminaotr. If she is loyal, she cannot be killed.
Fun Fact #1: If you mod your file to force Miranda to be devoured by the Seeker Swarms. She will Lazarus herself back for the next scene.
Fun Fact #2: If you successfully kill Miranda however have neglected Jacob's loyalty mission. Upon completion, she will appear in the scene afterwards as normal despite being dead.

This can be rectified by sending Mordin to accompany the crew back to the Normandy or simply replaying the mission and trying again. I advise leaving Garrus behind as well.
This is of course presuming you are certain your friend had all their loyalties.

Oops didn't realize you couldn't kill her at all before that. Man...she's got some mega awesome survivability!

I'd like to see a screenshot or photo of the squadmate screen. Doing all the loyalty missions doesn't necessarily mean having everyone's loyalty. I might sound like an ass about it, but I firmly don't believe in the idea of a random death scenario considering I've run the suicide mission hundreds of times and randomness never was a factor. No one's been able to prove it in any case.aimlessgun wrote...

They don't.
Ken lives. Gabby, Kelly, and Rupert die. Crewman Patel and Rolston are the two crew members in the crew quarters talking about Rolston's daughter, and it's Patel who lives while Rolston dies. Crewman Hawthorne and Goldstein are the two crew members in the mess, and both of them will die. I don't know why I know this.Planet_Side wrote...
