 Best Idea Ever
I would like to post this feedback discussion about upping a difficulty over
Insanity Mode, once played through insanity, have the ability to unlock (Pro Mode)
which adds twice the enemy's, up's the Ai's (with a Ai Director Program) to adapt to
the way you play, the way you maneuver around around levels.
Give the ai's the ability to use all talents and skills based on there set class.

As a reward for beating Insanity mode, you are given Pro Mode.
Once pro mode has been unlocked and has had atleast 1 playthrough,
you are given the ability to use 2 bonus talents and a new weapon/armor set,
that was made by (Cerberus).

Pro Mode Adds To The Following:

Credits - Around the same as insanity but adds 1000 more credits

Exp - Enemy's give 20% extra exp based on damage taken from that specific character type

Enemies - have slow auto-regenerating shields, health, armor.
More tech/biotic abilities for the enemies
at least: such as Damping, Overload, Drone, Incinerate
Stasis, Throw, Warp, Shockwave - By Kronner

Teammates - But squad AI would have to be a bit better (especially at taking cover), do the same damage as Shepard and have more health and shields. Shepard could probably use a lot more health and shields too (at least twice as much) - By Kronner

Gameplay - Bring back enemy Snipers
They can one shot Shepard. But Shepard is warned before the shot is fired (just like in ME1) and has just enough time to take cover - By Kronner

:Note:
Recommended For Those Who Can Handle Insanity but looking for a challenge.

Or another great idea is to get a mass accelerator weapon as the normandy's main gun. (coughs Me2)

Mass Accelerator Weapon - AKA (BFG)

AI can't be that good in a videogame. I'd rather see other tweaks for Insanity.

a) no auto-save
Death due to bug? Who cares, it is called INSANITY for a reason. Although proper QA would be needed.

B) Enemies with auto-regenerating shields and health
To make it a bit harder.

c) More tech/biotic abilities for the enemies
at least:
Damping, Overload, Drone, Incinerate
Stasis, Throw, Warp, Shockwave

d) Bring back enemy Snipers
They can one shot Shepard. But Shepard is warned before the shot is fired (just like in ME1) and has just enough time to take cover.

e) Add more enemies
Not much though, maybe 20-30% more.

f) Remove some of the cover available to the player
Some of the crates are unnecessary.

optional:
g) Add a +20% cooldown penalty to all Shepard's powers
On the other hand, reduce the squad members' cooldowns.

But squad AI would have to be a bit better (especially at taking cover), do the same damage as Shepard and have more health and shields. Shepard could probably use a lot more health and shields too (at least twice as much).

Someone mentioned there is a lvl6 difficulty that can be accessed via save editor... ppl can try that. Pretty 'Pro' sounding from what I heard.

I think it would be more beneficial to tweak the ratings of Insanity to make it harder rather than add a whole new tier of Difficulty. Or at least change the name of Insanity, if Insanity isn't the hardest its not that insane.

Some of the Biotic Enemies, Engineers and Lieutenants would recharge their barriers and shields as well if left alone long enough.

I wouldn't mind an AI that used powers more aggresively and used more of them, e.g. I want to fight a krogan vanguard with biotic charge. I'll probably be hitting reload pretty often, but the victory would be so much sweeter. Maybe give the enemies some grenades to throw at you if you camp out behind cover too long.

I do prefer a deterministic AI to a stochastic AI that adjusts to player tactics. Most of the time the tactics for these games are pretty simple and you can rig up a decision tree that would make pretty good choices. Introducing a stochastic element pretty much ensures occasionally the AI will do something stupid for the sake of learning/refining wieghts. I would like it if the enemies coordinated somewhat, e.g. units in cover lay down suppressing fire while other units advance and try to flank. It wouldn't have to be too complex, but flanking alone would make the game much more challenging.

I don't want the highest difficulty level to be unlockable though, I want to be able to play it on my first play through.

You could always do the Devil May Cry approach. Shepard Must Cry. Play the game with no shields ever or perhaps everything dies in a single shot.

If they made the AI actually aggressive, that alone would change everything in terms of difficulty. As it is right now, the AI only sorta flanks, and when it does, it's usually kind of slow at doing it. In fact, when the player advances, the most common AI response is to immediately backpedal, which is not always that smart for them. On the other hand, if all regular enemies had the same kind of behavior as Harbringer, combined with an improved enveloping behaviour, then even hardcore or veteran mode would become much more difficult, imo, as it'd effectively eliminate bunkering as an effective strategy in many parts of the game.

If only they allowed us to mod mass effect like dragon age. Heres to hoping Mass Effect 3 gives us some modding tools since its the last in the trilogy. Modding the game shouldnt effect the save files.
