What sells big here most likely wont sell big there or vice versa.
Here are examples:
Dynasty Warriors 7 - Japan is raving over it. American reviewers are already planning to rate it a 0/10 without ever playing it.
Call of Duty - Americans want every game of the series and plan to buy every release at full price. They don't care that the multiplayer is pretty much the exact same as the last entry and the Singleplayer is non-exisitant. These fans blast games like Dynasty Warriors 7 despite the fact that the Dynasty Warriors entries add more new content compared to the Call of Duty games.
Final Fantasy - Much bigger in Japan. Though they have grown to despise the series because of Square's failed attempts to grab and even larger Western Crowd. The truth of the matter is that they will never be abnle to appease both crowds justly in this manner.
Tidbits:
Western - Shooters are all the craze. Realism looks, high graphics, and lots of gore/blood.
Eastern - Giant Robots are all the craze. Fantasy setting and they desire to have games as an escape and not to be too realistic or western.

Not surprising. Nobody is going to worry about marketing over there either, it's just an uphill battle. It's a shame, really. J-RPG devs really need a few hints to revitalize the genre.
But hey, a japanese porn actress likes Mass Effect, country redeemed in my eyes

Ever read about the hoops that Microsoft had to go through to release the XBox in Japan and to poor sales? Hell, they're still selling poorly in Japan. The Japanese crowd is either in the Handheld scene or only the PS3/Wii scene.
Such as the PS3 sales of FF13 compared to the 360 ones in Japan.

You might ask "Why do the Japanese just lap up on such titles", well... why do Western Gamers love Call of Doodie when the Multiplayer changes ever so slightly throughout each 'new' title. You can't blame them for buying such titles because over here in the West people just chow down on the latest Halo, Doodie & over-the-top-steroid-abusing-generic-action-dude-game, and it's true! Oh well.

Something I haven't researched but that might play into the difference: how old is the average video game player? I've read somewhere that adults in their twenties are the largest player group today. Video games are no longer a childhood phenomenon. This might be impacting on the growing number of mature games. I don't know about Japan, to be honest, but I seriously doubt that all that many people in their twenties or older still play video games. Or rather, they don't play at home. They don't have the time. Hence the DS/PSP boom.
Aren't all video games an escape from reality?? To me it doesn't matter whether you play a gaudy fantasy game or Medal of Honor. Games always function as metaphors for real life problems. Characters looking like real human beings now does not mean the game depicts reality. Western games work with alternative realities and future realities, yes. Ironically ME is much more captivating than any JRPG I've played lately, thus drawing me into this fantasy future a lot better. Allowing me a better escape from reality ;D
You guys make the Japanese sound like freaks, you know that? It's not like all Japanese who play video games are into women with huge breasts looking like 12 year olds. The average adult Japanese probably doesn't much care for video games anyway.
And please, as if Miranda and Samara are not ridiculous looking characters! Half-naked Japanese characters? Morrigan, anyone?! It's just a different kind of sex appeal. I, as a woman, find 99% of all female characters from any video game ridiculous! (Male characters, too, for that matter.)

Mass Effect 2 was released in Japan long time ago on XBox 360 and PC.
XBox sales are very poor there and PC games sales aren't tracked anywhere.

And yes, I do think that a more mature title like Mass Effect would have more limited appeal since it's okay to be a nerd in America but in Japan, once you reach adulthood, you really need to leave those things behind since there's a huge stigma about being an otaku or worse, a hikikomori. Still, I would think that the level-up system and dating sim elements would allow the title to be a huge hit in Japan. Guess I was wrong.
Still, western gamers haven't picked up Blazblue: Continuum Shift which is an absolutely fantastic fighting game that has an excellent online mode. The Japanese still play Dynasy Warriors which is the same game every single time while also being far too long to hold me attention given that the gameplay is so shallow while brawlers have really moved beyond Dynasty Warriors in terms of gameplay. It's been years since Devil May Cry, God of War and Jade Empire and yet Dynasty Warriors still plays like it came out in 1998. Then you have the American market loving Call of Duty which is has been exactly the same since the first Modern Warfare, having jumped the shark back when World at War came out in 2008. Plus Call of Duty is a very noob-friendly game that lacks the depth of Halo: Reach or Unreal Tournament 3 and heavily favors camping while also giving the player automatic weapons that kill in one hit.

you also forgot the tall of their controller and how small the Japs hands usually are... on that point I can understand them turning on the DualShock 3, exact sames reasons why I hate Sony's pads.
