 I did everything "right" on the Suicide mission, yet Mordin died.
Is this inevitable?

No it isn't. I'm guessing you had everyone loyal, (or not since he died, which shouldn't be possible if you have everyone loyal and pick the right person for each task). The safest way to prevent him from dying is to have him loyal and then send him back to the ship with as the crew escort. That way he'll automatically survive.

Not a bug. At least, I'm pretty sure it's not unless you do the "Hold the Line" calculation and discover it isn't working as it's supposed to. If you're ready to have it all spoiled, this is a comprehensive guide.
