The achievement is to kill 500 darkspawn, my records indicate that I've killed 517. The achievement is still incomplete. Is this a known bug?

I do not know how well known it is, but I have the same problem. I have cleaned the Cache, reloaded the game, played it through several times, and cannot get that or the 1000 achievements to pop. the only thing keeping it from my all-time favorite game, I cannot get all the achievements.
