I'd appreciate some help for my sentinel playthrough on insanity. The fact that I haven't gotten the achievement on the xbox 360 has been bugging me for quite awhile. Any tips about what powers I should concentrate on, and what squadmates will benefit my class the most?

I prefer to go for the passive first. For a Sentinel I would choose Guardian for the lowest possible cooldown, since you'll be using powers all the time.
Next I'd evolve Tech Armor. If you play very aggressively, you might want Assault Armor. But for a "caster" type player, you should definitely take Power Armor instead. Sinc you haven't played on insanity before, I think Power Armor is the obvious choice in any case.
For the rest of your points, I would suggest maxing out Energy Drain (bonus power) and Warp, and Throw as the last one.
For squadmates, I think Miranda is always great to have for the group buff and Warp. I would take a squadmate with a squad ammo power with me as well. In the beginning, Jacob is invaluable for his incendiary ammo so you can keep the vorcha and krogan from regenerating.
Sentinel is probably the class with the most survivability because you can recharge your shields with Tech Armor. But you have to survive the first couple of missions where you have few points and no upgrades. That's by far the hardest part of the game. Particularly after Horizon, everything gets pretty easy.
