So I'm going through the story and I open a container and part of the loot is an Omni-Tool. The stats on it are 15/19/15 and the name of it is "V" or the Roman numeral for 5. It has no model (eg Chameleon) nor manufacturer. I have heard that there are certain items that cannot be obtained through normal means, only through the PC cheat console. I thought that was limited to armor and weapons, but is this item one of those? Did I glitch something somehow to get an item I wasn't supposed to?

This is a pretty common glitch. Don't worry, you didn't bug anything.
I think you should be fine to go ahead and use it if you want, but if you want to be sure you can always omni-gel it.
