Prior to getting the First Enchanter's Cowl I use Helm of Honnleath.  Only 90% Dodge until then though.
Every level 2 Magic, 1 Will.
I take Arcane Warrior first and just melee through until level 14 (with the belt that increases xp from codex on an ally, it is a quick attain)
Nature immune mobs, either Bear Form or just pop sustainables and melee with Arcane Warrior.
Even with auto hit attacks from bosses and such, I have never had a problem at all with mana and sustaining Flying Swarm.  This is on Nightmare difficulty aswell.  I will be trying a solo version in the future going Shapeshifter first since most of this equipment is low str requirements except Cailin's Greaves.  Duncan's Dagger is just filler for the extra Master Dweomer Rune since Spellweaver only has 2 rune slots.

Shapeshifter
------------------
Flying Swarm: 40% dodge
The Spellward: 10% dodge, 30% spell resist
Cinch of Skillful Maneuvering: 10% dodge, 10% spell resist
Key to the City: 4% spell resist
Cailin's Greaves: 20% dodge
Ancient Elven Gloves: 4% spell resist
First Enchanter's Cowl: 10% dodge, 4% spell resist
Reaper's Vestments: 10% dodge,  16% spell resist

Weapons
--------------
Spellweaver: 10% spell resist
Duncan's Dagger: 24 Dex requirement

Runes
---------
Grand Master Dweomer x1: 10% spell resist
Master Dweomer x2: 16% spell resist

Why do you go SS (magic) and AW?... nothing wrong with it but it does seem a bit redundant.

p.s.

In my experience 'Flying Swarm' does not need any dodge but would benefit more from magic regeneration and fire resistance.

AW for Cailin's Greaves. And having 100% dodge with 100% spell resist in Flying Swarm is win.

Keep in mind that Ogre's Ram attack, and Golem's Slam attack will hit you and can't be dodged, so an Ogre can force you out of shape with a few Ram. There are probably more special attacks that is a guaranteed hit but I can't think of any other than the two I've mentioned.

Cept for those auto hit moves. Kind aredundnat though since your damage is pitiful.

Its actualy a better build that was discussed last year that has a Shapeshifter mage put practicaly all points into Str, build a 100% dodge rate and then maul everything in Bear Form for a nearly unstopable killing machine.

Shapeshifter/Arcane Warrior is only good if you go all magic stats wise so that all your attributes are boosted in animal forms higher and so taht you cna use magic to equip better armors. Even then you can make a practicaly unhitable character by stacking defense items alone as a AW.

People say that the damage is pitiful in Flying Swarm and that a single Ogre will force you out of it.
In my experience hitting everything around me for 20-30 damage every second and being able to move extremely fast around the map is pretty good for me. Also, I've fought the two Ogres in the Dalish woods and even with them both beating on me with their auto hit attacks, I had enough mana to just sit there and take it. With a 100% dodge bear you wont have 100% Spell Resistance. Sometimes I think people post before they think. With my build, my magic is still very high and even in Bear form I'm hitting for around 50-60 damage per Overwhelm attack, as well as having 100% spell resistance and still 60% Dodge rate.

It is just a different build and when people criticize it without bothering to test it, it makes me laugh a bit. .

You can take criticizem many differant ways.  I just happen to prefer the builds that for example kill those same two ogres in that encounter in seconds. No need for damage immunitys if your target is dead.

*EDIT*
Also I think the 100% Dodge, 100% Spell Immune, All Armor Bypass Warrior build is another build similar to this.

*EDIT 2*  Hmmm I wonder if its possible or how close you can get a to a Bear Form with 100% Dodge/Spell Resistance.
