When I hold an instrument and use an Inspiration my character plays the instrument. If he is not holding an instrument he just sings. Does holding the instrument give any advantage or is this just a roleplay thing?

You mean, even if it's a magical instrument they don't have to hold it to gain the benefit of the enhancement that is, by it's very nature, a part of playing the instrument?

Pretty much. An instrument with special abilities can still be activated from the quickbar. At least it worked that way last time I played a bard. That was years ago. A patch could have changed it. The only time I used to switch to an instrument was during easy fights when I felt cocky. 
I particularly enjoyed the variant sounds of playing curse song out of a flute or lute as opposed to singing it.

That's true for wands, but don't you need to equip an instrument before using its special abilities, like, say, a ring? I never use instruments for this reason (weapons are better to deal with enemies), but maybe I'm doing it wrong.

Instruments had to be equiped to use their special abilities the last time I checked. They just aren't necessary for bardic abilities. Which is okay by me. I don't see a reason why singing couldn't be just as valid as playing an instrument.

In all my time playing a bard I never recall coming across an instrument that gave a bonus to the bard feats/skills/songs/bonus songs. Shame that. Maybe they are out there but haven't played NWN2 in a while so little fuzzy on it.

I have never seen anything that had any direct effect on bard abilities. Typically magical instruments just get on-activation spell effects and/or a bonus to the perform skill (which does indirectly impact the bardic song). To use the spells, the item has to be equiped. I would assume the same for the bonus to perform but I am not sure.
It would be interesting to see magical instruments that impacted bardic abilities in more fundamental ways.

There is at least one instrument that gives a bonus to Perform (ask Grobnar for a hint). This does have an effect on Song of Requiem and Curse Song.
