Not in Baldur's.... ah, well, your foster father. Though he is not really a controlled team member.

Baldur's Gate II? Only vaguely remember, but I think, while a lot people died between the two games, no one actually dies... only Imoen disappears. Might be wrong though... I only remember that you wake up in a prison and there is this "Relief" guy. Ah, the morbid, black humor those days.

Not in Neverwinter Nights. A lot of people die around you because the academy you're training at is being attacked, but your only team mate survives and decides to give up on adventurinig.

Knight of the Old Republic.... hmmm. You wake up on an Galactic Republic military vessel while it is under attack. Someone wakes you up, but I don't remember who. Was it Carth? Or some red shirt who didn't survive? I think it was Carth, in which you wouldn't loose a team mate.

In Mass Effect evidently yes. In Dragon Age... yes... if you count the battle at Ostagar as opening phase. But they don't really die in battle. However, if you count the background part as opening not. In my case - female city elf - my best friend got raped, several people I knew died but no team mates....

No comment on Dragon Age II, too young, don't want to bring spoilers here...

If it isn't already James Sanders (which really seems likely to me actually), I hope it's either Jacob OR Miranda or both, depending on their status and/or if you romanced one of them.

Why? It's not because I hate either character.

It's because of maximum possible impact. You need to seperate Shepard from any serious Cerberus connections at the beginning of the game (even partially if you seriously side with them in ME2), and some big event has to go down. Well, one way to help solidify a Cerberus seperation is to kill off the two most important Cerberus operatives who would likely vouch for Shepard the most Jacob and/or Miranda.

Now it would be cruel of BW to kill off a possible continued love interest, so if you romanced one in ME2 and it stuck, then that character should obviously survive. But if you didn't romance either: boom! Dead, make way for new NPCs, and a reason to add Cerberus antagonism once again, regardless of how you treated the organization in ME2.
