Just finished my second play through and apparently I now have this achievement, don't know how I got it, there was no notification via a little box in the top left hand corner and no reference to it from any character or cutscene. I'm callin' bugs, this game needs some serious patching. (Merrill's Act 3 quest especially cos it's f*cked inside out)
