Let me descrive what I did that worked for me...
I set the dificulty to insane before starting a new game, when I did started the new game the insane dificulty was already selected and I went through the game without touching it. Note that I did change to other (careers) walkthroughs that had diferent dificulty setting and played them while I still hadn't finished the insanity playthrough and that still didn't affect me getting the insanity trophy at the end.
I guess as long as you don't touch the dificulty setting for the insanity playthrough after starting it it's ok to switch back and forth between careers.

Yep - Changing careers has nothing to do with the Glitch.

You know, Bioware DON'T have to do this patch, so how about you show
a little appreciation that they're actually patching this up instead of
moving on to mass effect 3 or something. God I hate ignorant fans.

Ummm, yeah...  a glaring oversight from Bioware somehow equals me being an ignorant fan.  Way to bring the conversation to the lowest common denominator...

Forgive me for wanting to hold a multi-million dollar corporation to a higher standard.  Moron.

Just had it unlock for me, whew. I followed the directions and did not touch the main menu difficulty prior to starting a new game. I just booted up and went directly to new game. The difficulty was already set to Insanity when the prompt came up.

The week prior to this I did change the difficulty to Insanity at the main menu and manually set Insanity on the previous career's saves when everyone was trying to figure out how to prevent the glitch. Just boot up the console, get in the game, and change the difficulty to when the prompt asks you to when you begin a new game.

Again, it is just a little lame because the trophy suggests that you need to play the ENTIRE game on Insanity. For me, I assumed the entire game would also include the interactive comic. After all, you make decisions in that part of the game, so one can assume that setting the difficulty to Insanity prior to starting the interactive comic would do nothing to create a glitch. Either way, setting the difficulty prior to actually starting, or when prompted should not matter...

I just beat the game on Insanity and I didn't know the trophy was glitched if you select the difficulty through the game options before you started a new game. When you guys patch this, will you be looking at the completed saves to register the trophy? Or will I have to play through the game on Insanity again?

I guess it was too much to expect Bioware to release the patch along with Arrival... Should have known better than to expect them to fix what they started BEFORE going for the dlc cash grab...

*sigh*

I'm going to post what I did to get Instanity trophy. I changed the difficulty setting to insanity from the main menu. Then I started a new game and picked Insanity difficulty when the prompt appeared. From what I understand the difficulty setting you picked is stored in your profile save file, not in your character save. In order to get the trophy you have to remember that once you start to play on Insanity, you have to finish the game with that character before you can play any other character. If you load a save of another character then the game might think you changed the difficulty setting and you won't get the trophy.

Jesse just said a few posts above that this does not affect whether or not you get the insanity trophey at the end. The glitch has to do with setting insanity at the options menu. So just aviod the options menu and set it to insanity before the prologue and you should be fine. He said changing careers has nothing to do with it even if they are set at a lower diffculty as other people have gotten the trophey when they have done just that.  Just wanted to clear that up and i am going for this now guys a little more then half way through wish me luck on beating it and a glitch free trophey lol!

Yeah, GFX1989 didn't read the thread. Many of us didn't have more than the one character on our profile. This is about changing the difficult pre new game. Those of us that already did this are waiting to see if the patch fixes our current saves. If not we have to start over.

Good luck, Dragonfan. I know the feeling of putting in the effort and time into the game on insanity and not see that trophy pop up. It was a really horrible feeling. I hope it doesn't happen to you too.

Thanks I hope it dosent happen to me either! lol yeah that would be a terrible feeling and sorry you had to experience it. Hopefully when the new patch comes out you just have to beat the final boss and the trophy will pop so that you don't have to do it all over again.

So this happened to me, completed my insanity run through and no trophy pop, particularly annoying as I had hit all the other trophies in my first play through naturally so I thought i could run through quickly (still took me 25 hours!) on insanity for the platinum, but nothing!

I don't know why it didn't work, perhaps because I selected insanity from the main menu before beginning the game or perhaps because I switched career mid-game to play the DLC on my first character, either way I'm surpirsed such a major bug got through testing, considering this game is quite old now if you include the XBOX/PC version.

I have patched the game now, and I'm sure I already know the answer to this already, but...  Is there any way I can get this trophy without having to play through all the way again (I doubt I'll find time to ever do another full run through)?  I have a save point from during the final mission on insanity.  I'd bet this isn't possible but I can hope huh?!

It's a massive dissappointment as I loved the first playthough, and enjoyed the second, although it became repetitive towards the end so I don't think I'd enjoy a third playthough at all.

It is worth a try but Jesse said they could not guarente it would work for save files that had been started before the patch. I am at the collectors base now on insantiy without the patch and once i get time to finsih it i am praying that it pops for me or i will also be extremely disappointed. Oh and if you set it to insanity through the main menu before starting that is why it glitched as switching to different careers has nothing to do with it but if you switch it in the main menu first it gets confused and thinks you changed the diffculty mid-game.

I started my playthough before the latest patch, but finished it after the patch, and I didn't get the trophy so I guess the patch deon't really fix the problem unless you are starting a brand new game from scratch post-patch.

It's even more annoying to realise that the reason I didn't get the trophy is because I chose to set the difficulty to insanity before starting the playthough rather than leaving it on normal and then changing it to insanity after the prologue as this seems to be the opposite of what the trophy description tells you to do!

Sorry that happened to you! I just finished my insanttiy run a few hours ago and it was before the new patch so it didnt really help me and i got the trophey and the plat! I am so happy it didnt glitch! If you were able to do it once i am sure you will beat it again it is unfortuante that you have to do it again though i would be very upset if that had happened to me. Good luck to you once you start your insantiy run again at least next time you wont have to worry about the glitch with the new patch now out.

I played through the first time on Normal. I then, at a later date, started the console, went directly to new game and when prompted before starting the prologue, selected insanity. I do not have the newest patch installed. Will I get the trophy when I finish the game, or must I install the patch and start over?

You should get the trophey when you finish the game as that is exactly what i did and got it so you should be good to go. With the newest patch you would not have to worry at all but it screws up the comic. You should be fine though even without the patch if you are waiting for the comic to be fixed. Well best of luck!

BioWare, I'm furious!

I've just completed my 2nd playthough, this time on "Insanity" difficulty, and my it was difficult! And guess what, the trophy didn't pop!!!

I know positively that I started out with the "Insanity" setting from the beginning, and I've not a single time entered the options menu before completing the game just to make sure that I've not spoiled anything and glitching the trophy!
I've NOT loaded another char but kept on playing my new char on the Insanity setting, so why the heck doesn't the trophy pop?!?!

The only thing that has happened during playing was that the new patch (1.03?) installed the other day, but surely that couldn't destroy my hard earned trophy?? BioWare, you really need to do something about this!!

Truly and utterly disappointed! What are we supposed to do when we can't rely on your trophy logics. and will you resolve this situation?

Mass Effect 2 doesn't have enough trophies imo. This is my last one to get and I have a feeling I'm nowhere near down with the game. Let's hope when I do my run that it works fine.
