Because we're coming at in from our own cultural context where a woman takes her husband's name and keeps it after he dies.
It's no different than the people playing female PCs that thought BioWare had been sloppy with the gender variables because they kept on being called 'Sir.' Or people who are surprised when Chantry Sisters can have sex and Templars can get married.

In setting, I think that's a fair point. And actually now I'm curious.I don't even think they have Ms, Miss, Mrs in Thedas, I don't recall a woman ever being referred to as such.

Mistress
Formal titles:
Ser <- Might also be a respectful, informal title
Sister
Bann
Arlessa

But heck, my mother still gets my father's mail for some god awful reason. So I shouldn't be too surprised.
