Anyway, the locations to each piece is below from personal experience playing as a warrior.
SPOILERS AHEAD, READ AT YOUR OWN DISCRETION
Gloves: Awarded after being named Champion
Warplate: On the body of a High Dragon in the Bone Pit
Boots: On the elf bloodmage that Meredith tasks you to kill or bring in. Located in the Alienage.
Helm: Acquired after rescuing Bethany from the Templar/Mage integral group on the Wounded Coast

That are most likley not the one from the set since you dont visit the deeproads anymore in the last chapter.
Champion gloves - Automatically received at the start of Act 3.
Champion chest piece - Defeat the High Dragon for the quest "Mine Massacre".
Champion boots - Taken from Huon in the quest called "On the Loose".
Champion helm - Taken from Grace on the quest called "Best Served Cold"
The set itself hasnt the best stats, compared to other single items. The Setboni evens that out a bit. But if you entchant it propper its not only the best looking set, but also the best overall. Anyone who fought the Highdragon on Nightmare knows how important resistances are, regardless of class.

It is a set of black armor that looks like it either has the GW insignia or the insignia of the Viscount of Kirkwall. It does look really good, but IMO it pales in comparison to the Champions armor. The Champion should look distinguished, not like the common armored soldierfragelrok wrote...

I got the boots from "On the Loose" on the 360 version but never got the gloves that everyone says are automatically given for starting Act 3. Another reason to play on the PC, could just dev-console them in so it would avoid the bugs.
Edit: I just lost 30 minutes of game play going back to the fight to end Act 2 and the gloves appeared this time.
