I expect it's within a single playthrough, as in the achievement is rewarding the player for consistently siding with one side or the other.  Like  you'll have to play through twice to get both "sides with the Templars" and "sides with the Mages" achievement - I expect the latter exists.

No, you can choose between Circle/dead in Deep Roads/alive. Somehow.

I'm thinking Bethany and Carver aren't permanent companions. I bet Carver chooses to join the Templars or Wardens after the expedition, depending on if you took him with you or not... Maybe he ragejoins the Templars if you don't take him, but is inspired to join the Wardens after visiting the Deep Roads. Bethany can apparently die on the expedition, and if not is taken to the Circle.

It was mentioned in a video I saw that it was a plot point whether or not your sibling goes with you to the Deep Roads. Perhaps Carver/Bethany joins or is forced to join the Circle or the Templars if you don't take them with you to the Deep Roads?

take them to the deep roads = grey wardens

leave them = templars or circle mage

Hmm... From what I found out It seems Merideth is the last boss.. It said "Merideth end game fight".

If he "dies" at Vigils Keep he gets saved by Justice and if he lives he gets possesed by Justice either way

Possibly, and If not were wrong one of the review said about Carver took role as antagonist. If he were Templar and Hawke fully side with Mage rebellion?

Then again...to be sure, I'll take him to Deep Roads:happy:
