DA2 has flags within its char creator scripting to make preset #1 and #2 uneditable. Within chargenmorphcfg.xml you can manually copy the preset to either two additional slots, or have them overwrite two other presets. You can then edit the presets themselves, changing the skin to your pleasure.
I would suggest doing this: copy the presets to two additional slots, not taking up any other presets. Start a game using the default face, as to not screw up your family's appearance. Then go to the Black Emporium and change your face to the added preset, modify as desired.

This XML adds the presets at the end, like I said, and also companion's hair, default Hawke hair/beard.
Like I said, DO NOT use the last two presets (editable defaults) at the start of the game, or else your family will look screwed up. The game's scripting defines premade faces to your family depending on what preset you use. Example: Did player use preset 3? -> Use this face for Bethany. The game's scripting effectively takes into account the first 11 presets (2 uneditables and 9 crap presets), so arbitrary 12th and 13th presets will yield a screwed up result for family. Do as I said, use preset #1 or 2 so your family looks normal, and then modify at the Black Emporium afterward. You can then use the editable Hawke preset and change the complexion to a smoother skin.
