The vessels aren't entirely sterile. They can't be. Nothing larger than a few rooms could be kept entirely sterile, especially while it contained living beings. They don't just stay on their ships - they have to trade, complete missions, and every single one is sent out of the fleet to bring back resources.
No, they're microcosms because they catch infections during their Pilgrimage/Missions. If they catch a virus from a Turian, that virus could stay with them anywhere from 6 months to eternity. Simply being decontaminated doesn't eliminate infectious agents from the blood.
Except that quarians don't actually catch
foreign diseases and thus can't be carriers. They just have allergic
reactions to them. And, again, any quarian returning to a Flotilla ship
would invariably have to go through decontamination prior to boarding so
any contaminants they bring with them would be eradicated. So what
practical use then is wearing the envirosuits aboard those vessels?
They can catch anything that would affect a Turian, who share amino-acid chirality.
Their allergic reactions are to L-formation amino acid bacteria and virii. That is, Human, Asari, Volus, Hanar, Elcor, Krogan, Vorcha, and Batarian.
