What does it take to be the Ultimate Champion? You must complete DA2 as:

Male Hawke

• Warrior
• Rogue
• Mage

Female Hawk

• Warrior
• Rogue
• Mage

Your brother and sister must each become a grey warden at least once.

You must romance each companion once: Isabela, Fenris, Merrill, Anders and Sebastian.  (how you choose to do that is up to you)

You must side with each faction once (list may grow after the game is released): 

• Templars vs. Chantry
• City Guard vs. Qunari
• Elves vs. Former Werewolf

What's the prize? No, not bragging rights - or at least not JUST bragging rights.
