Attack is simply your chance to hit, while Damage is exactly what it sounds like. And since this is in the spoiler section anyway, may as well list all of the tomes and locations.

Rest assured, I will be heavily involved in discussing Nightmare related topics in a few days. But, as a little teaser, I'll say that I typically play as a Warrior and only used ranged companions (for reasons made obvious by your video)
NOW that has me excited! I always love playing outside the box tanks, that don't use shields.
Shields are for warriors that are trying to hide. I swing my two-hander with extreme prejudice
Seriously though, I like Shields and Big weapons.

Not to sound flippant, but how far into the game are you? I haven't found anything yet that requires kiting on Hard. So far, most of the game has been so easy that my Warrior could pretty much solo everything by himself other than the true bosses if I wanted to just down health potions all of the time. This is especially true in any of the fights within the city, or caves that have narrow passages. The AI is so bad that all you need to do is herd them with line of sight and then start hitting 5-6 enemies per swing (yeah, I know what I said earlier, and that's still true if you don't abuse the AI).
The difficulty was also actually lowered for all but Nightmare, in which now it's actually a moderate challenge sometimes.

I'm by no means "pro" but your vanguard guides and videos for Mass Effect 2 bring out a new level of fun in the game that I doubt I would've been able to without. Right now I'm playing through as a "trailer" mage Hawke on normal for the story and fun, but I look forward to trying this out my next playthrough. I imagine it'll be as much of a night and day difference as it was for me in Mass Effect.
Look forward to picking the fruits of more of your playthroughs.

I don't see how it's possible without pausing as you need to constantly be shifting the group around to avoid the ARW, and one small mistake can wipe half or more of the party. Fight took me 20 minutes because of his insane HP/defense and all the micromanagement.

It works a little differently on Xbox. It gives you all the points for the respec, but doesn't remove any of your spent ones. Doubling your current ability and specialization points.

I'm waiting until you get to the Ancient Rock Wraith to see if your no-pause strategy is viable. The first two dragon bosses are pushovers compared to it

Petrify did save my ass. Unfortunately the music I attached to this video (indestructible) is copyrighted. Thats the reason I was gonna have that song for this fight.

Kept aggro by unloading on him, and with Varric constantly using Goad.

I also like bring him with Anders since I get friendship with anders and rivalry with fenris almost every dialogue.
I've hear if later on you side with one specific side and Fenris is a rival, he may leave.

I've been playing on hard and I'm enjoying it. Its not as challenging as DAO was; but its not a cakewalk like Normal is. I'll probably up it to Insanity for a second playthrough; but right now my tactical RPG skills are a little rusty so hard is still keeping me entertained.
I just wish that on hard you could get friendly fire from spells; but not melee attacks. I liked having to aim my spells carefully in DAO (even though that was a challenge playing on 360); but with a 2 handed warrior anything near me is going to die and there isn't much I can do to conveniently keep my melee allies from getting smacked.
Does anyone know if there is a way to program the AI of a melee wielding ally to stay the hell away from my whirling dervish of death warrior while still engaging enemies?

Yikes, I play on 360 and I was thinking of giving this a go; but that's a little extreme. I'm down with a little cheating; but doubling my ability and specialization points is some pretty hardcore cheating.
