"Commander" is a common enough naval and often air force rank. And Shepard is a member of the Alliance Navy.
In any case, talk of sergeant / major etc. is irrelevant as no such rank exists, in and of itself, in (I believe, any English-speaking) navy.

When Shep takes command of the Normandy she's still the rank of Commander but she gains the title while on board the Normandy of Captain, every ship has a "Captain" and that officer can have an official rank of Lieutenant but still have the title of Captain.
There is no automatic promotion upon taking command of a vessel.
