Because he's dieing. He doesn't have enough time to be like "oh hey I remember you!"
Why would they need to make more then one demon that can turn into a mouse? A more logical explination is thus:
Mouse warns you that Sloth is very powerful and dangerous. The only reason he doesn't bother to attack is because he's currently disintrested -- Sloth tells you as much.
Later on, at the circle tower, sloth has been placed inside a mage's body, and now he's not disintrested ;p
Mouse is a pride demon, yes, but just because pride demons in general are powerful, doesn't mean this one in particular is very tough. He's been feeding mage initiates to a rage demon, as per the conversation about how he was to weak to do it himself and needed the help.
Thus, mouse is a-not-as-powerful-as-he-could-be pride demon, who gets slaughtered by sloth (or any other number of the lesser demons helping him). You encounter him and he's all "OMG I'm dieing. Here. Turn into a mouse and get that SOB for me. Thx."
SEE, perfectly logical.
