Part of the change is down to the new game engine. For all we know, his DA2 look is how he was visualised all along and they've only just been able to make him look the way he's supposed to be. Don't get too bogged down in trying to give game mechanics story-based explanations. Sometimes you just have to shrug it off and accept it.

I noticed early someone had asked if there was a playlist for Anders that people played... and as I was reading that a song by Adele came on and I thought it perfectly fit the relationship of Anders and Hawke by the end of the game. Granted I only romanced him playing the friendship level, but I'm sure it's even more fitting if you Rivalmanced him.

but really; the enemies exploding into bits (especially when varric kills them), the 4 man army against two dozen qunari and winning, the parts where varric isn't actually there (such as the love scenes)... all made up in my book! but it does allow a lot of head cannoning *invents naughty scenes*

I always assumed the game fades to black before any actual sex because Varric was unwilling to talk about it with Cassandra. He might fill in some of the details of the post- and pre-sexytimes (almost just had a Sheldon moment and used the technical "coitus," but I always feel a bit like I'm in bio when I hear that word), but he can't bring himself to imagine the full details. Or maybe he can and Cassandra stops him.
Personally, I like to imagine Varric has Isabela show him how to sneak into the manor for his viewing pleasure. We already know he embelishes Hawke's journal. What else does he do when he's there?
