That's funny. The first time Anders activated Vengeance mode with me, it was more or less the same thing. Came completely out of the blue.
The only thing I can think of is that I must have hit it accidentally. Although... now that you mention it, my Hawke was near death, too.

But I could probably set it up in Anders' tactics to use Vengeance when Kaya's being attacked... not that I've tried to use the tactics in DA2 yet.

Anders's default tactics is activating Vengeance when attacked or something like that, no? It would work even if the spell is not on the quick bar.

I have no idea. I've actually never looked at the DA2 tactics, and I'd have to look up how to use them in the instruction booklet. xD I also didn't try out the tactics in Origins until I had the game for over a year.
