I went ahead and ran through DA:O, Awakening, and Witchhunt...in that order, making some very specific choices to see if they would show up.

Once into the game I did runscript zz_vault_debug and discovered the following were not active but had been intentionally chosen in the game.

I romanced and slept with Morrigan but not flagged. I freed the Magi Circle but instead it said I gave land to Dalish. It flagged Anders as dying at Vigil Keep but he did not. It has Avernus doing both good and evil research when I only gave him the option to do good. My character also left with Morrigan at the end of Witch Hunt but saw no mention of that..I don't know if that is something that gets flagged.
