I feel utterly silly about this but I just finished defending the City of Armaranthine and it went pretty well but somehow Vigils Keep fell and Velanna, Justice, Oghren, and Varel died. Did I not get all the upgrades?
I pay'd for the upgrades, found all the stones to upgrade the armor, found the granite, found trademen's, and the supplies to update the bombing items?
Can anyone please tell me what I missed?
Was I suppose to do all of Wade's additional armor quests? Or did I miss something?

I placed them to defend the farmers
does the codex usually say this about everyone being dead?

You can save everyone but Justice if you do it right.

Did you actually play till epilogue?
The game always says that the Keep has fallen and all defending companions are dead right after saving the city, but after beating the last boss you'll find out that everything is ok with the keep and companions (if you fully upgraded it and got the achievement for it).
