I had JUST completed broken circle, so hearing another desire demon speaking out of an innocent kitty almost made me jump out of my chair. I think I literally yelled "Oh, ****!" in real life.

My first encounter with the Lady of the Forest, the spirit who was guiding the Werewolves, was so great, I loved the design.
Also creepy is the way the Desire Demon's touch themselves when they talk to you.
Oh, and the Orphanage, with the children's voices and the toys lying scattered, and those war hounds penned up in the rooms filled with gore...felt like max Payne scene

To me the most disturbing thing about the game was the voice right before the brood mother and her creepy poem. That stands out as the most disturbing.
Speaking seriously, yes I agree with that...
...the first play-through, I remember by that point in the game saving Orzamar for last, then being in those damn tunnels forever, then finally hearing that voice... Ugh, it was creepy!
No it isn't that either. The creepiest/most disturbing thing is the Alienage Orphanage Poem.. That whole damn place was creepy as hell.
"One, two, Maric's run through"... that poem.

Actually, I felt I little left down by the Orphanage side-quest... And the villains are just more enemies you've already seen. (globulous rage demon types)
I was HOPING that strange Fade beast demon that I assembled, summoned, and released ("Asunder" quest, deep roads) would have returned and was devouring children's souls or some henous thing. But no, just more rage demons to kill. Not so bad IMO.

Same for me. Probably didn't help that I was playing it in my basement at about 3am.

Also titled: "So about the mighty Sandal... WHAT THE HELL DID HE DO?"
When you're near end-game, and you go into that room just littered with wrecked Darkspawn bodies...and then SANDAL is standing amidst the carnage...just smiling...and looking...as though nothing is amiss...
WHAT THE HELL???
Did the Darkspawn try to take one of his runes, and he pulled out some hitherto unknown Dwarven stone-magick, and just annihilate the lot of them?
Is that why he's a savant? A Dwarf somehow made magical, and it messed with his mind?
My 2nd play-through was as an 'evil' Blood Mage, making every horrible choice throughout the story doing everything short of eating babies (thought the Orphanage would give me the chance, but alas, no)... Yet have to say observing Sandal in that room wigged me out more than anything else I experienced!
I don't see why you think it's magic. He clearly went on a rampage. I love the idea of him just shoutiing "ENCHANTMENT!" the whole time.

Day two... Dang, I can't remember. Hrm... Was it, "They make me eat my shoe?"
Hrm... No... That couldn't be it. In any case, broodmother was creepy as ****.

No, some people with downs syndrome don't respond to pain correctly, so they can overexert themselves and not care. That's the extent of it.

Sandal is just awesome He probably had a lyrium overload and did a massive AOE like Riddick can. Or he was Chuck Norris in disguise.

Second day, they beat us and eat some for meat
Third day, the men are all gnawed on again
Fourth day, we wait and fear for our fate
Fifth day, they return and it's another girl's turn
Sixth day, her screams we hear in our dreams
Seventh day, she grew as in her mouth they spew
Eighth day, we hated as she is violated
Ninth day, she grins and devours her kin
Now she does feast, as she's become the beast
