How do you upgrade them? I honestly have no idea and I don't have the guide. Any help would be quite nice

Does their armor change physical appearances like Bethany's does after you apply the upgrades? I hit max friend status with Bethany right after the Birthright quest (more specifically after I gave her the portrait of mother) and noticed she got the awesome looking chainmail armor. I'm in act 2 currently and while I've upgraded some of the companions armors, the appearance hasn't changed....
