So I managed to activate all my bonus content and received the relevant quests in game. The special deliveries chest in Hawke's house has the quest marker on it but when I clicked on it, I only received the +2 to attributes book and nothing else.
Now the quests are still in my log, the chest is still marked as a quest object, but I can't even click to open it.

Seems I can't edit my post. Just wanted to add that I tried save and loading, and going to a save before I first looted the chest. Neither solution worked.

In the game's main menu
Under "Extras" Check "Unlockables" see if the items are listed. If not then it isn't linked correctly to your EA account. If they do show up then I have no idea what may be wrong.
