Has anyone manage to find all of them? So far I've found:

1. Chantry 2nd floor to the left.
2. Outside Vicount's audience chamber
3. Abandon Theig at Sundermont
4. Cave at Wounded Coast

6. Recently opened passage in Sundermount

Your missing one is definitely the bone pit one. Once you have all 5 you have to go back to Darktown for the final confrontation.
