The Chantry: South side of map on a table.
Viscount's Keep: East side, throne room, on a bench before the stairs.
Bone Pit: East side, middle path, in a cave that is rather small.
Sundermount: West side, near exit, inside a cave. Lots of undead and some Golems. Book is at very end of the dwarven cave.
Wounded Coast: North-west side, the cave is near a fireplace with some crates that have qunari markings on them, on the path leading south. Cave has a dragon at the end, and it's rather tough for being so small.
Final tome - Darktown: Forgotten Lair, north side of darktown in it's own area. Desire demon (Xebenkneck) at the end, with a lot of rage demons.
From what I have read, if you read any of the first 5 you can't complete the quest, so you must destroy the 5. The final tome (as I have come to find) can be destroyed, read, or taken like the previous. The difference is that the quest will complete regardless of which you choose. Reading the tome and accepting the offer (since it's the first time) grants 2 attribute points. Taking the tome does nothing, and destroying it just finishes the quest. I personally read the final tome. Save before you read the final tome, in case you don't like the results.
edit: Oh, and Xebenkneck drops a rather nice blood mage staff if you
destroy the 5 to access the 6th. The loot from Xeb seems to be the same
regardless of what you chose to do with the final tome (tested
3/10/11).

Did you read any of the dialog when you picked up the first tome? Or the letter you recieve which activates this quest. I imagine if you did you wouldn't be posting this topic.

It explains that you can't read any more once you read one but if you kill her you can still find the tomes but you don't get the letter to explain anything
