I know this trophy says in a year (an act) dont get the player or companioins knocked out. I just finished Act 1 on Normal. During the playthrough when someone died I would just load the autosave and ensure noone would die when beating that part...does this not work because the trophy didnt unlock or maybe it was due to bethany dying during the Prologue but I assume that is not part of Act 1....or is it?
 
I just would like some concrete guidelines on how this is done

I don't know really but having the prologue count as act one sounds believable.
